Learning disability services: user views on transition planning
This study provides insights into attributes of continuity of care valued by people with a learning disability. Possibilities of translating these attributes into practice w ithin localities are explored. Findings could be used to inform strategic planning locally to promote service integration thereby contributing to continuity of care within transition planning.
