The Potential of Transexamic Acid Use for Wide Awake Local Anesthesia No Tourniquet
Summary: Tranexamic acid (TXA) is a drug commonly used to decrease intraoperative bleeding. Its use in various types of surgery is well defined; however, its use in plastic surgery and even more so, hand surgery is not as well researched. Concurrently, the use of wide-awake local anesthesia no tourniquet (WALANT) procedures has increased in popularity. As the use of WALANT increases, it comes to question whether TXA could be used as an alternative to epinephrine during WALANT procedures. This case involves a 33-year-old woman status-post motor vehicle collision. On examination, the patient sustained transection of the ...

Finger Abscess Caused by a Black Fungus, Exophiala xenobiotica
SUMMARY Exophiala xenobiotica is a relatively new species of black fungi that can cause infection that primarily affects the skin and soft tissues in humans. Black fungal infection is challenging to diagnose and often needs surgery. However, few studies have reported black fungal and E. xenobiotica infections in the field of plastic surgery. Herein, we report the case of a 79-year-old man who presented with a finger abscess, later identified via rDNA sequencing as E. xenobiotica infection. He did not have a history of immunosuppression. We resected the lesion and performed skin grafting. No recurrence was observed, eve...

Dupuytren Disease: A Retrospective Cohort Study Comparing Collagenase Injection and Percutaneous Needle Fasciotomy
Conclusions: The results of this study show that PNF appears to be as effective and minimally invasive as CCH injection, but at significantly lower cost. Considering these factors, the authors prefer and recommend the use of PNF over CCH. (Source: Plastic and Reconstructive Surgery – Global Open)

Nonpalmar Endoscopic versus Open Trigger Finger Release: Results from a Prospective Trial
Conclusions: Patients treated for trigger finger with a nonpalmar endoscopic release through an incision at the proximal digital crease demonstrate significantly better scarring in the early postoperative period compared to patients treated with the open surgical approach. Treatment for trigger finger with this technique is as effective as the standard open technique. (Source: Plastic and Reconstructive Surgery – Global Open)

Sexual Dissatisfaction after Hand Surgery
Background: Using the hand questionnaire (HAND-Q) patient-reported outcome measure, the effects of upper extremity surgery on patients’ perception of their sex life were explored. The hand is a uniquely sexual organ, and we hypothesized that self-reported measures of disease severity, quality of life, and emotional impact would correlate with sexual dissatisfaction among patients receiving treatment for hand/upper extremity conditions. Methods: Patients were prospectively enrolled for hand questionnaire participation. Patients with valid responses to the following questions were included: functionality, hand a...

Plastic Surgery in the Gulf Cooperation Council Countries: Role of Telemedicine
Conclusion: The tendency toward telemedicine integration in plastic surgery practice is growing worldwide, especially after COVID-19. Its limitations are outweighed by its advantages and, with time, many of these will be bypassed. New innovations driven by advancement in technology will further embed telemedicine into the practice. Orbital Exenteration and Brachytherapy for the Treatment of Pediatric, Fusion-positive, Recurrent Rhabdomyosarcoma
SUMMARY A 20-month-old boy presented with biopsy-proven recurrent alveolar rhabdomyosarcoma without metastasis. He was previously treated with multiagent chemotherapy and external beam irradiation showing a complete response. Upon relapse, he was treated with chemotherapy, orbital exenteration, and brachytherapy. Customized, intraoperative brachytherapy has potential to limit the sequelae associated with radiation adjuvant therapy. (Source: Plastic and Reconstructive Surgery – Global Open)
