Diffuse Type Neurofibroma of the Forearm
We present a rare case of diffuse type neurofibroma found in the forearm, presented to our clinic as a slowly enlarging mass of the left forearm of 3 years duration. The lesion was suspicious in the magnetic resonance imaging, and biopsy revealed diffuse type neurofibroma. We opted for total excision of the lesion that was found to be not possible due to involvement of the major nerves. The final pathology report showed no malignancy. Nerve tumors of the upper limb can be either benign or malignant. Neurofibroma associated with neurofibromatosis has malignant potential. The diffuse type is rare, and it most commonly occurs...

Brachial Plexus Birth Injury: Trends in Early Surgical Intervention over the Last Three Decades
This study seeks to determine trends in the early surgical management of BPBI over the last 30 years. Methods: A systematic review was performed through MEDLINE (PubMed) identifying studies limited to the early surgical management of BPBI from 1990 to current. Patients treated after 1 year of age (ie, tendon transfers and secondary reconstructive efforts) were excluded. Diagnostic tests, age of intervention, surgical treatment modalities, and outcome scoring systems were extrapolated and compared so as to determine trends in management over time. Results: Seventeen studies met criteria, summating a total of 8...

Candida albicans Flexor Tenosynovitis after Trigger Finger Release: A Case Report
We report a case of an immunocompetent patient with finger flexor tenosynovitis caused by Candida albicans after trigger finger release (TFR). Intratendon sheath triamcinolone injection and subsequent TFR surgery might contribute to the occurrence of Candida tenosynovitis. We performed a tenosynovectomy twice because the pathology was unclear after the first tenosynovectomy. The culture of synovial tissue from the first tenosynovectomy was negative. However, the culture from the second tenosynovectomy tested positive for Candida albicans. We must consider the possibility of infection with low virulent pathogens such as fun...

Release of Severe Postburn Contracture of Index Finger with Two-stage Reconstruction
Summary: Postburn contractures are the result of deep burns that are not treated properly and promptly. Severe contractures require more than one surgery to be completely relieved. Their surgical treatment is very difficult and challenging for reconstructive surgeons. The author presents a case with severe postburn contracture of the index finger especially at the DIP joint with an angle of 110 degrees, which was released with a full skin graft in two stages of reconstruction. The application of this procedure enables the complete release of severe contractures and protects the fingers from their loss as a result of ...

Surgical Treatment Outcome of de Quervain’s Disease: A Systematic Review and Meta-analysis
Conclusions: Five percent of patients still have residual pain after surgical release of the first extensor compartment. Surgery type, as well as the type of incision, did not affect outcome or complication. Thus, surgical release of the extensor retinaculum for DQ disease is an effective treatment, regardless of the type of surgery. (Source: Plastic and Reconstructive Surgery – Global Open)

Infection Rate Comparison during Transition from Hospital to Office WALANT Enabled by Virtual Reality
Conclusions: This narrative study concurs with the current literature that WALANT in the office setting is as safe as the hospital OR-based procedures for selected elective cases. By transitioning suitable cases from the OR to the OPR, a surgeon’s overall infection rate should not change. (Source: Plastic and Reconstructive Surgery – Global Open)

Prophylactic Desexualizing Mastectomy for an Intellectually Disabled Woman: Protective Measure or Disregard for Autonomy?
We describe a 19-year-old intellectually disabled woman who presented with her legal decision-maker for surgical correction of her asymmetric chest and hypoplastic breast. The decision-maker requested a mastectomy to make the patient’s breasts as inconspicuous as possible, and to reduce the risk of sexual assault and pregnancy. This case covers the ethical considerations on whether or not to prophylactically desexualize an intellectually disabled woman. (Source: Plastic and Reconstructive Surgery – Global Open)

Normative Values for Adolescent Quality of Life in Plastic Surgery: A Longitudinal, Cohort Study
Conclusions: Adolescence is a challenging time, on which plastic surgery-related concerns are superimposed. Our findings suggest that girls may be more at risk for developing psychosocial deficits that worsen over adolescence and young adulthood. This observation is critical for the interpretation and contextualization of health-related quality of life in adolescent plastic surgery patients. Necessity of Temporal Artery Biopsy for Giant Cell Arteritis: A Systematic Review
The objective of this study was to determine the usefulness of TAB for giant cell arteritis management. Methods: We performed a systematic review to identify studies that compared steroid treatment between TAB+ and TAB− patients. EMBASE, MEDLINE, and the Cochrane Central Register of Controlled Trials were searched from inception until April 4, 2020. Titles, abstracts, and full texts were reviewed by two independent reviewers and conflicts resolved by consensus. Studies reporting TAB result and steroid treatment were included. Information pertaining to steroid treatment was compared between TAB+ and TAB− groups. ...

Gender-affirming Mastectomy: Comparison of Periareolar and Double Incision Patterns
Conclusions: Our results demonstrate no difference in the rates of postoperative complications or revision procedures based on surgical technique. These results also suggest that with an experienced surgeon and proper patient selection, both techniques of gender-affirming mastectomy can be performed safely and with comparable outcomes. Disparities in Research during Plastic Surgery Training: How Can We Level the Playing Field?
This study evaluates the extent of this challenge for plastic surgery trainees and identifies potential solutions. Methods: Data were collected on academic plastic surgeons' research productivity during training. Bivariate analysis compared publication measures between genders and race/ethnicities at different training stages (pre-residency/residency/clinical fellowship). Multivariate analysis determined training experiences independently associated with increased research productivity. Results: Overall, women had fewer total publications than men during training (8.89 versus 12.46, P = 0.0394). Total publica...
