Recent Advances in Endovascular Neurosurgery: Pushing the Envelope
Neurosurgery has historically been a field that embraced rapid technological advancements. Endovascular neurosurgery is no exception to this. As we move away from traditional routes of neurosurgical therapy, we will continue to push the envelope toward less-invasive and more-innovative treatments. Syndromic Hydrocephalus
Hydrocephalus, the abnormal accumulation and impaired circulation/clearance of cerebrospinal fluid, occurs as a common phenotypic feature of a diverse group of genetic syndromes. In this review, we outline the genetic mutations, pathogenesis, and accompanying symptoms underlying syndromic hydrocephalus in the context of: L1 syndrome, syndromic craniosynostoses, achondroplasia, NF 1/2, Down ’s syndrome, tuberous sclerosis, Walker–Warburg syndrome, primary ciliary dyskinesia, and osteogenesis imperfecta. Neurovascular Syndromes
Patients with cerebrovascular syndromes are at risk for additional concerns associated with their syndrome. A wide variety of syndromes are associated with cerebrovascular diseases. Multidisciplinary care is helpful to ensure comprehensive evaluation and management. Precise diagnosis and appreciation for the underlying syndrome is critical for effective cerebrovascular and broader care. Syndromic Neurosurgery: Pulling It Together
In this issue of Neurosurgery Clinics of North America, we are excited to explore the growing field of syndromic neurosurgery. While neurosurgeons have long cared for patients with cranial and spinal syndromes, we increasingly see organization of the specialty within and across institutions. One of the primary challenges, both in clinical practice and in reviewing these topics, is the shear breadth of a field defined by such divergent syndromes. Neurosurgical Evaluation and Management of Patients with Chromosomal Abnormalities
Patients with chromosomal abnormalities are at risk for numerous neurosurgical pathologies, given the broad impact and multisystem involvement of these disorders. Down syndrome (trisomy 21), Edwards syndrome (trisomy 18), Patau syndrome (trisomy 13), Klinefelter syndrome (47,XXY), and velocardiofacial or DiGeorge syndrome (22q11.2 deletion) are particularly associated with neurosurgical concerns. Given the heterogeneity of concerns and presentations, these patients benefit from multidisciplinary care provided by teams familiar with their specific syndrome. Safety and Quality in Syndromic Neurosurgery
The developing field of syndromic neurosurgery has significant challenges and opportunities in quality and safety. Quality care must be safe, effective, patient-centered, timely, efficient, and equitable; the Donabedian model focused on system structures, processes, and outcomes is a helpful framework to guide improvement in these areas. Ultimately, a successful syndromic neurosurgery program will bring together an interested multidisciplinary team of experts who will grow care through open communication and steady improvement efforts.
