Novel Treatments and Clinical Research in Child Neurology
The molecular understanding of the pathogenic mechanisms responsible for neurologic diseases of children has led to a remarkable period of research that addresses the root causes of diseases. The promise of this research has been realized with cures and treatments that correct underlying deficiencies. The breakneck rate at which new research is being proposed promises to usher in a transformation of child neurology from a diagnostic and supportive field into an interventional one. Training child neurology residents in clinical research and therapeutic intervention is increasingly important to assure the ongoing ability to ...

Genetics in Epilepsy
This article provides an overview of the major classes of genes associated with epilepsy phenotypes divided into functional categories along with the recommended work-up and therapeutic considerations. Gene discovery in epilepsy supports counseling and anticipatory guidance but also opens the door for precision medicine guiding therapy with a focus on those with disease-modifying effects. Memento Akademia: Introduction and Editorial Regarding the State of Child Neurology
Memento Mori, “remember death,” was whispered into the ears of victorious generals in ancient Rome as a reminder of their mortality and fallibility. The title of this article is meant to summon similar reminders, but of an academic mission. This mission is difficult to sustain in current health care systems, thus making this call timely and important. (Source: Neurologic Clinics)

Practical Concepts in Electromyography
Electromyography has been a longstanding valuable technique in the evaluation of neuromuscular disorders. The basic concepts of nerve conduction studies and needle electromyography were well established decades ago. While these same concepts still form the basis of the technique and interpretation of the studies today, advances in the techniques, approaches, and findings recorded during testing have helped physicians more accurately identify diseases. Furthermore, while advances in other ancillary testing, such as genetic testing and neuroimaging, have provided useful methods to diagnose neuromuscular disorders, electrodia...

Epilepsy Surgery
Drug-resistant epilepsy warrants referral to an epilepsy surgery center for consideration of alternative treatments including epilepsy surgery. Advances in technology now allow for minimally invasive neurophysiologic monitoring and surgical interventions, approaches that are attractive to families because large craniotomies and associated morbidity are avoided. This work reviews the presurgical evaluation process and discusses the use of invasive stereo-electroencephalography monitoring to localize seizure onset zones. Minimally invasive surgical techniques are described for the treatment of focal and generalized epilepsie...

Inflammatory Diseases of the Central Nervous System
Pediatric neuroinflammatory conditions are a complex group of disorders with a wide range of clinical presentations. Patients can present with a combination of focal neurologic deficits, encephalopathy, seizures, movement disorders, or psychiatric manifestations. There are several ways that pediatric neuroinflammatory conditions can be classified, including clinical presentation, pathophysiologic mechanism, and imaging and laboratory findings. In this article, we group these conditions into acquired demyelinating diseases, immune-mediated epilepsies/encephalopathies, primary rheumatologic conditions with central nervous sy...

Pediatric Neuro-Oncology
Central nervous system (CNS) tumors are the most common solid tumor in pediatrics and represent the largest cause of childhood cancer –related mortality. With advances in molecular characterization of tumors, considerable developments have occurred impacting diagnosis and management, and refined prognostication. Advances in management have led to better survival, but mortality remains high and significant morbidity persists. Nov el therapeutic approaches targeting the biology of these tumors are being investigated to improve overall survival and decrease treatment-related morbidity. Further molecular understanding of ped...

Neurology of Sleep
Healthy sleep, including proper amounts in the 24-hour day/night period, is crucial for developing children. Sleep development in infants and children is characterized by increased amounts of sleep, including rapid eye movement and non –rapid eye movement (NREM) slow-wave sleep. Expected changes as well as deviations may contribute to sleep problems, which are common in typically developing children and very common in those with neurodevelopmental disorders and often are chronic. Periodic screening of children for sleep problems is important for timely and effective management of these. (Source: Neurologic Clinics)
