Exploring the Link Between Thyrotoxicosis and Cognitive Decline in Older Adults —Reply
In Reply We thank Fu and colleagues for the opportunity to clarify the design of our study, preventing a misreading that is critical to the interpretation of the work. Fu et al raised concern that imperfect adherence to prescribed thyroid hormone may have led to an overestimate of “the risks associated with prescribed doses of exogenous thyroid hormone.” However, the exposure in our study was not prescribed thyroid hormone, but rather thyrotoxicosis as measured by thyrotropin laboratory values. Importantly, the 1.39 adjusted hazard ratio cited by Fu et al is the associati on measured between cognitive decline and all-c...

Exploring the Link Between Thyrotoxicosis and Cognitive Decline in Older Adults
To the Editor In the pivotal cohort study by Adams et al, an in-depth evaluation of the association between thyrotoxicosis and cognitive decline in older individuals was conducted. Analyzing data from 65  931 individuals 65 years and older within the Johns Hopkins Community Physicians Network (2014-2023), the study discerns a critical association between reduced thyrotropin levels and elevated occurrences of cognitive impairments, including dementia. Notably, the study identified a 1.39-fold incre ase in cognitive disorder risk in patients with thyrotoxicosis, spotlighting the need for measured thyroid hormone prescripti...

Improving Women ’s Health Across the Life Span
Even though most patients of general internists and internal medicine specialists are women, our knowledge of women ’s health and illness is remarkably sparse. Women’s health is often conceptualized solely as reproductive health without consideration of health issues throughout the life span. Moreover, persons who are pregnant or could become pregnant are often excluded from clinical trials so that health car e choices for women of reproductive age are typically extrapolated from clinical research that was performed mostly among men. Women face complex social determinants of health, due to their disproportionate roles ...
