Care Demands Ahead of Transitioning Into Residential Care
In the US, family and unpaid caregivers are the primary source of assistance to older adults with care needs who are living in the community. In addition, paid services and supports, such as personal care and household assistance, may augment help from family and unpaid caregivers. Residential care settings —including assisted living and long-term nursing facilities—most often serve older adults with the most substantial care needs. (Source: JAMA Internal Medicine)

Solving the Kidney Transplant Conundrum Through Systems Thinking
Kidney transplant is the optimum treatment for individuals with kidney failure because it provides significantly improved quality and length of life compared with dialysis. Live donor kidney transplants offer the best outcomes compared with deceased donor kidney transplants and dialysis. Despite this, kidney transplants are underutilized —not only because of a shortage of deceased donor kidneys, but also because of the lack of access to and underutilization of live donor kidneys. Machine-Made Empathy? Why Medicine Still Needs Humans —Reply
In Reply Establishing a strong physician-patient relationship, which includes patients feeling heard and understood, is essential to health care. An automated tool that creates empathic language offers promising opportunities for bolstering the physician-patient relationship. We found that responses generated by an artificial intelligence (AI) chatbot were 9.8 times more likely to be considered empathetic or very empathetic than responses authored by a physician. Moreover, this difference persisted even when we compared the chatbot replies to the longest physician-authored replies. (Source: JAMA Internal Medicine)

Further Analysis Needed or Was It Lead Reversal —Reply
In Reply We appreciate the letters from Kawji and Chang et al regarding our report of migrating localized ST-segment elevation in the setting of acute pericarditis. In response to Kawji ’s comments, we agree that the ST-segment dynamic change in this case was fast. This was seen on the repeated electrocardiogram (ECG) in the emergency department (Figure, B), obtained 25 minutes after the first ECG by emergency medical services (Figure, A). It continued to evolve with migrating ST -segment elevation in localization as seen on the third ECG (Figure, C) obtained 88 minutes after the first ECG, and was followed by diffuse ST...

Further Analysis Needed or Was It Lead Reversal?
To the Editor I read the interesting case report by Xiao and Zhou. It is very unusual for the intense inflammation of acute pericarditis to resolve in 1 area in a few minutes, contrary to what is suggested by Xiao and Zhou. Rather, the inflammation may spread to other areas as mentioned in the case report. As correctly reported by the authors, stage 1 of ST-segment elevation in acute pericarditis persists for at least a few hours. (Source: JAMA Internal Medicine)

Amplifying the Strength and Leadership of Nurses in Opioid Use Disorder Care
From the Civil War to the COVID-19 pandemic, nurses have been at the very heart of the response to the US ’s greatest public health crises. The unique set of skills instilled through nursing education, combined with great courage and a commitment to the care and comfort of the patient, has not only made nursing care indispensable during these periods, but has also propelled nurses to the forefront of leadership and innovation. For example, programs such as Boston Health Care for the Homeless and the first HIV specialty ward, founded at San Francisco General Hospital in 1983, were each co-led by nurses at a time when fear...

Gender Equity in Clinical Trial Leadership
Gender disparity in clinical trial leadership has been described previously in fields such as cardiology. In this issue of JAMA Internal Medicine, Waldhorn et al demonstrate that this issue is more pervasive. They look across clinical trials of different medical specialties and report a persistent underrepresentation of women serving as principal investigators. This disparity was shown to exist in nearly every field other than rheumatology. (Source: JAMA Internal Medicine)
