Novelties in Therapy of Chronic Heart Failure
In recent decades, considerable advances have been made in the treatment of heart failure. The main target of heart failure therapy is the inhibition of the sympathetic nervous system and renin-angiotensin-aldosterone system. The angiotensin receptor blockers represent a breakthrough in the treatment of heart failure with a demonstrated effect on reduction of cardiovascular events. However, new perspectives derive from latest drugs developed for diabetes, iron deficiency, and hyperkalemia. New frontiers are also opened to the development of neurohormonal therapies, antagonists of inflammatory mediators, inotropic agents, a...

Exercise-Based Cardiac Rehabilitation Programs in Heart Failure Patients
Exercise training is recommended for patients with heart failure by major societies ’ guidelines. It improves exercise capacity and quality of life, reduces symptoms of depression, can improve survival, and reduce the risk for hospitalizations. Exercise-based cardiac rehabilitation can be offered with different modalities, such as continuous or interval aerobic training, resistan ce, and inspiratory muscle training. The intervention must follow an accurate evaluation of the patient’s cardiovascular conditions and functional capacity. Despite the multiple benefits of exercise training, there is a lack of adherence to ex...

Role of New Technologies in Supporting the Treatment of Complex Patients
Cardiology represents one of the privileged disciplinary areas for the experimentation and validation of the applications of telemedicine. Telemedicine, and the health technologies that go by the name of eHealth, identify the digital exchange of social and health information in order to support and optimize the care process remotely. Telemonitoring applied to cardiovascular diseases is defined as the recording, remote transmission, storage, and interpretation of cardiovascular parameters and diagnostic images. Meta-analyses have shown that telemedicine-supported models of care not only are effective but also cost-effective...

Biomarkers in Heart Failure
Heart failure (HF) is a clinical syndrome caused by structural and/or functional cardiac abnormalities and resulting from impaired cardiac output or an increase of intracardiac pressures at rest and/or during stress. Typical signs and symptoms of HF include ankle swelling, fatigue, dyspnea and peripheral edema, pulmonary crackles, or increased jugular venous pressure. Usually, patients with ejection fraction (EF) greater than or equal to 50% are defined as HF with preserved EF, whereas those with EF less than 40% have HF with reduced EF. Combined Effect of Mediterranean Diet and Aerobic Exercise on Weight Loss and Clinical Status in Obese Symptomatic Patients with Hypertrophic Cardiomyopathy
We evaluated the impact of weight loss (WL) using a Mediterranean diet and mild-to-moderate –intensity aerobic exercise program, on clinical status of obese, symptomatic patients with hypertrophic cardiomyopathy (HCM). Compared with nonresponders, responders showed a significant reduction of left atrial diameter, left atrial volume index (LAVI), E/Eʹaverage, pulmonary artery systolic pr essure (PASP), and a significant increase in Vo2max (%) and peak workload. Body mass index changes correlated with reduction in left atrial diameter, LAVI, E/Eʹaverage, PASP, and increase of Vo2max (mL/Kg/min), Vo2max (%), peak workload...

Stage A Heart Failure
Stage A heart failure (HF) patients do not show HF symptoms or any structural heart disease but are at risk of HF development. Cardiovascular risk factors (hypertension, diabetes, metabolic syndrome, sedentary lifestyle, poor diet, and exposure to cardiotoxic agents) characterize subjects affected by stage A HF. It is essential to identify these subjects early and ensure that, despite being asymptomatic, they grasp the importance of undertaking correct lifestyle and therapeutic interventions. A careful stratification of asymptomatic subject ’s risk profile is needed to adopt proper preventive strategies and to set indivi...

Heart Failure: One, None, and a Hundred Thousand
Heart failure represents an important public health problem worldwide, with a broad spectrum of clinical scenarios, starting from primary prevention in an asymptomatic phase to acute and chronic congestive heart failure, with use of modern drugs and devices, rehabilitation, and telemedicine strategies.1 –3 So, we can appropriately paraphrase the renowned Italian writer and Nobel Prize recipient, Luigi Pirandello, and his famous novel, written in 1926, as: “Heart Failure: One, None and a Hundred Thousand.” Diagnosis and treatment of heart failure are particularly difficult and challenging, esp ecially in these dramati...

Role of Cardiac Magnetic Resonance Imaging in Heart Failure
This review describes the current role and potential future applications of cardiac magnetic resonance (CMR) for the management of heart failure (HF). CMR allows noninvasive morphologic and functional assessment, tissue characterization, blood flow, and perfusion evaluation. CMR overcomes echocardiography limitations (geometric assumptions, interobserver variability and poor acoustic window) and provides incremental information in relation to cause, prognosis, and treatment monitoring of patients with HF. Asymptomatic Left Ventricular Dysfunction
Stage A and B heart failure (HF) include asymptomatic patients without and with structural cardiac disorder, respectively. Asymptomatic left ventricular (LV) dysfunction represents an early stage of HF that should be recognized to prevent overt HF development. Echocardiography plays a pivotal role in assessment of cardiac structure and function and represents the ideal imaging technique for screening in the general population, thanks to its availability, feasibility, and low cost. Traditional echocardiography, with LV systolic and diastolic function and cardiac remodeling assessment, is usually performed. Development of ne...
