The Vital Role of the Endoscopic Oncologist
In 1993, I was recruited to be the Head of Gastrointestinal Oncology at the University of California, Irvine within our new NCI-designated Comprehensive Cancer Center. Prior to accepting this position, I interviewed a few prominent figures who worked in this unique space where gastrointestinal (GI) intersects with Oncology —and I came to the conclusion that this was indeed an area of clinically unmet needs. This was further confirmed by my father’s diagnosis of locally advanced colon cancer and the journey we would travel together as a family. However, magnified endoscopic examination with image enhanced endoscopy (IEE-NE) is becoming increasingly important for qualitative diagnosis of GI neoplastic lesions. Endoscopic Ultrasound-Guided Antitumor Therapy
Endoscopic ultrasound (EUS) has been used for various interventions to manage intra-abdominal lesions. EUS-guided antitumor therapy via delivery of chemotherapeutic agents, energy, and radioactive seeds has advantages of less invasiveness than surgical approaches, and the anatomic proximity allows easy and accurate access to the pancreas. The feasibility of EUS-guided antitumor therapy has been reported both in pancreatic solid and cystic neoplasms, with promising preliminary results. Endoscopic Management of Malignant Biliary Obstruction
Endoscopic retrograde cholangiopancreatography (ERCP) is commonly used for managing malignant biliary obstruction; however, it is impossible if the endoscope cannot reach the ampulla of Vater, and it carries a risk of procedure-related pancreatitis.
