Endoscopic Stenting for Inflammatory Bowel Disease Strictures
Strictures are among the most frequent complications in patients with Crohn's disease (CD), usually requiring a combined medical, surgical, and/or endoscopic approach to treatment. Currently, endoscopic balloon dilation (EBD) is the endoscopic treatment of choice, but its effectiveness is not universal, especially in the long term, and it is not free of complications. The technological evolution of stents in recent years has allowed their use in benign diseases of any origin and location, including inflammatory bowel disease (IBD). The current scientific evidence regarding the use of stents in strictures in IBD is limited ...

Porcine Model of Anastomotic Stricture in Crohn ’s Disease
The development of new endoscopic methods is advancing rapidly. Current standard methods such as endoscopic balloon dilatation have only limited long-term effects. Surgery is more effective, but it also carries a higher risk of complications. Endoscopic stricturotomy and stricturoplasty are new methods expanding the range of possibilities of endoscopic treatment. These methods are advanced, technically demanding, and require adequate expertise and training. It is, therefore, necessary to have a tool for training and teaching these new methods. The live large animal model is a valuable tool in the development and testing of...

Interventional Inflammatory Bowel Disease: Endoscopic Management of Complications
A majority of patients with inflammatory bowel disease (IBD) will eventually develop some form of structural complication, ranging from strictures to fistula and colitis-associated neoplasia. Management of structural IBD requires a multidisciplinary approach. Endoscopic therapy plays a bridging role in the medical and surgical management of structural IBD, providing a more definitive treatment than medical therapy and less invasive treatment than surgery. Endoscopic management of IBD complications has evolved into a subspecialty; that is, interventional IBD has evolved into incorporating principles, doctrines, technology, ...

Overview of Common Complications in Inflammatory Bowel Disease Surgery
The complication rate after surgery in patients with inflammatory bowel disease is high owing to chronic inflammatory and suboptimal physiologic state, the effect of steroids and immunosuppressive medication, and the inherent complexity of the surgical procedures. Although some of the complications after surgery are similar for Crohn disease and ulcerative colitis, others are specific to the diagnosis. Complications are divided into early postoperative and late complications. Specific complications are related to more extensive surgery such as a proctocolectomy or reoperative procedures or with complex reconstructive proce...

Image-Enhanced Endoscopy in the Surveillance of Colitis-Associated Neoplasia
Advances in endoscopic technology have allowed for improved detection and management of dysplasia. These developments have also raised the question of the optimal methods for surveillance. Promising data showed that virtual chromoendoscopy (VCE) is comparable to dye-based chromoendoscopy (DCE). However, the usefulness of DCE and VCE in the surveillance of longstanding inflammatory bowel disease colitis when compared with high-definition white-light endoscopy has been recently questioned. Confocal laser endomicroscopy is a highly innovative endoscopic procedure but is still far from the routine adoption for surveillance. Th...

Interventional Endoscopy Has Changed Practice in Inflammatory Bowel Disease
The first decades of the twenty-first century have seen tremendous progress in the management of inflammatory bowel disease (IBD). Medical and surgical treatments have both advanced, but with a foot in both these specialties, interventional endoscopists have established a major role in the treatment of IBD complications. Endoscopic therapy has been applied in particular to strictures associated with Crohn disease, ulcerative colitis, and postoperative anastomosis. Fistulas and abscesses have also been successfully managed in many cases, avoiding more invasive surgery. (Source: Gastrointestinal Endoscopy Clinics of North America)

Endoscopic Therapy for Fistulas and Abscesses in Crohn's Disease
Crohn disease (CD) patients can develop fistula or abscess from persistent active disease or postsurgical complications. Penetrating CD is traditionally treated with medication and surgery. The role of medication alone in the treatment of fistula is limited, except perianal fistulas or enterocutaneous fistula. Surgery is the standard treatment in those with hollow-organ to hollow-organ fistula, like ileovesicular fistula. Surgery is invasive with a higher risk of postoperative complications. Endoscopic therapy has evolved as a valid option. Fistulotomy, surgical or endoscopic, should be considered first-line therapy when f...
