Vascular Plug Embolization of a Superior Mesenteric Arteriovenous Fistula: Case Report and Literature Review of Endovascular Treatment
Arteriovenous fistula (AVF) rarely occurs in the portal venous system. Aetiologies include iatrogenic, surgical, and penetrating trauma of the abdomen. Clinical manifestations of superior mesenteric portal arteriovenous fistula (SMPAVF) are right heart failure, mesenteric ischaemia, or signs of portal hypertension. (Source: EJVES Extra)

Focal Aortic Dissection With Significant Stenosis: A Rare Long Term Complication After TEVAR for Blunt Traumatic Aortic Injury in an Adolescent Patient
Thoracic endovascular aortic repair (TEVAR) in children and adolescents after blunt traumatic aortic injury (BTAI) is increasingly being performed, despite no endovascular graft being approved for TEVAR in this population. The smaller diameter of the aorta and access vessels and steeper angle of the aortic arch pose specific challenges in TEVAR in this population. Moreover, data are lacking regarding medium to long term complications. This case presents an adolescent patient who underwent TEVAR for BTAI and suffered a focal aortic dissection several months later. (Source: EJVES Extra)

Intra-Operative Occlusion of the Contralateral Common Iliac Artery by the Gore Excluder Iliac Branch System
During stent grafting, managing the internal iliac artery (IIA) becomes a significant issue when an abdominal aortic aneurysm (AAA) is complicated by bilateral common iliac artery (CIA) aneurysms. The iliac branch system (IBS) has a defined length; therefore, the CIA should be sufficiently long. However, situations arise where the IBS must be used even in patients with a short CIA. Herein, a case of contralateral CIA occlusion due to deviation of the proximal iliac branched component of the IBS is reported. (Source: EJVES Extra)

Ilio-Iliac Arteriovenous Fistula due to Aorto-Iliac Aneurysm Rupture
A 73 year old male patient with history of ischaemic cardiopathy and diabetic nephropathy presented with acute left lower limb oedema and dyspnoea. Venous ultrasound found a patent deep venous system, but retrograde pulsatile flow was noted in the left common femoral vein. A computed tomography (CT) scan diagnosed a 56 mm infrarenal abdominal aortic aneurysm and a 44 mm right common iliac aneurysm (CIA) with a fistula to the left common iliac vein. He was treated with a bifurcated endograft landing on the right external iliac and left common iliac arteries, with previous embolisation of the right internal iliac artery. (Source: EJVES Extra)

Lobar Torsion Following Open Repair of Type B Aortic Dissection and Thoracoabdominal Aortic Aneurysm in a Patient with Marfan Syndrome
Introduction: Lobar torsion is a rare, challenging diagnosis that requires a high index of suspicion and prompt investigation and management. Detection and urgent fixation or lung resection are critical to avoid catastrophic sequelae of lung necrosis, bronchopleural fistulae, and mortality.Report: A case is presented of lobar torsion following open repair of a type B aortic dissection and thoracoabdominal aortic aneurysm in a patient with Marfan syndrome. After a non-specific constellation of symptoms, the diagnosis was confirmed with computed tomography and bronchoscopic findings and the patient underwent detorsion and pl...

The Role of Near Infrared Spectroscopy in Diagnosing Stump Ischaemia in Patients with Below Knee Amputation: Case Reports
Functional near infrared spectroscopy (fNIRS) can be used to quantify stump oxygen saturation (SaO2) as one of many possible causes of pain following major amputation. Although commonly used for cerebral perfusion monitoring during carotid or cardiac surgery, it can also be used to monitor tissue perfusion in the lower limb and predict healing post below knee amputation (BKA). The aim of this study was to measure the SaO2 trend as there is no validated oxygen threshold to diagnose insufficient flow into the arterial collateral network currently. (Source: EJVES Extra)
