A Rare Case of Upper Limb Phlegmasia Cerulea Dolens
Objective: Phlegmasia cerulea dolens is a rare clinical entity that manifests as a swollen, blue, painful limb. It most often occurs in the lower limb and is associated with iliofemoral deep vein thrombosis. Its occurrence in the upper limb is even rarer. The authors present a rare case of upper limb phlegmasia cerulea dolens in a patient with homozygous Factor V Leiden. (Source: EJVES Extra)

Carotid Artery Stenting and Improvement of Cognitive Function: Systematic Review and Meta-Analysis
Objective: Carotid artery stenosis is defined as “asymptomatic” in the absence of associated acute cerebrovascular events in the previous six months. Several studies have demonstrated that such stenosis might not be truly asymptomatic, since patients with asymptomatic carotid stenosis frequently have co-existing cognitive impairment. Studies h ave assessed the effects of carotid revascularisation on cognitive function in patients with asymptomatic carotid artery stenosis, with conflicting results. (Source: EJVES Extra)

Contemporary Results of EVAR in the Treatment of Ruptured Abdominal Aortic Aneurysms
Introduction: Abdominal aortic aneurysm (AAA) rupture is associated with significant morbidity and mortality. Endovascular aortic aneurysm repair (EVAR) has emerged as a safe and effective treatment for ruptured AAA (rAAA), being now recommended as the first strategy for anatomically compatible aneurysms. Outcome of Single Stage Hybrid Revascularization for Multilevel Peripheral Arterial Disease
Introduction: Multilevel disease is often seen in peripheral arterial disease (PAD) Simple management options are challenging. Hybrid procedures, combining endovascular and open surgery, is an ideal strategy for managing this cohort of patients.(1) The aims of this study were to investigate the suitability of hybrid surgery in those with multilevel PAD. Evolution of the Inflammatory Markers in Patients with Chronic Limb Threatening Ischemia
Introduction: Peripheral arterial disease (PAD) affects more than 202 million people worldwide, 1.3% of which with chronic limb-threatening ischaemia (CLTI) [1,2]. The primary cause of PAD is atherosclerosis, characterized by an intense inflammatory process [3]. Inflammation is associated a higher mortality rate, particularly in patients with CLTI [3]. The purpose of this study was to compare the evolution of inflammatory markers between patients with claudication and those with CLTI over time. (Source: EJVES Extra)

Careers in Vascular and Endovascular Surgery in a Country Lacking an Integrated Vascular Residency Program
Introduction: Vascular and endovascular surgery represents one of the most needed specialties in medicine, particularly in regions that lack an integrated vascular residency program. Indicators in the current market, such as long waiting periods for surgery or clinic appointments, suggest that the current demand for vascular surgeons surpasses the available supply. Unlike general surgery, medical students spend less time in vascular surgery rotations and have limited exposure, which can hinder their ability to make informed decisions about perusing a career in this field. Arterial Access Complication in Patients Undergoing ECMO – A Retrospective Analysis
INTRODUCTION: The usage of extracorporeal membrane oxygenation (ECMO) has been increasing in recent years. Veno-arterial ECMO (VA-ECMO) is frequently employed in the setting of heart failure, and the common femoral artery is the most used arterial access. Arterial access complications are known to occur and can have significant impact on these patients. Through this study, we aim to analyze the incidence of arterial access complication in VA-ECMO in adults, associated risk factors and Vascular Surgery interventions. (Source: EJVES Extra)

Could Inflammation be the Key to Understand Peripheral Arterial Disease?
Introduction: Skeletal muscle (SM) and adipose tissue are the two largest organs in the body with an endocrine action in several metabolic processes, including atherosclerosis[1]. Obesity directly contributes to the development of cardiovascular risk factors and cardiovascular disease[2]. However, some obese patients are metabolically healthy, underscoring the complexity of adipose tissue biology[3]. The function of SM in atherosclerosis is not as well understood, although some of the molecules produced by this tissue protect against endothelial injury by inhibition of oxidative stress, reduce the severity of aortic athero...

The National Varicose Vein Study: Portuguese Multicentric Prospective Study Regarding Surgical Treatment of Varicose Veins
Introduction: Chronic venous disease (CVD) affects 35% of the Portuguese population, similarly to other western world countries, and is responsible for over one million lost working days, 21% of job changes and 8% of early retirements. Endovenous techniques emerged as alternatives to open surgery and are currently the standard of care for treating lower limb varicose veins due to their efficacy and safety profile. In Portugal, however, due to the greater experience and easier access to this treatment modality, open surgery is still the most used approach. (Source: EJVES Extra)

Effectiveness of Endovascular Management of Chronic Pelvic Pain in Pelvic Congestion Syndrome Type I
We present our first prospective data collection for embolization of incompetent refluxing gonadal veins in women suffering from pelvic pain at Ain Shams University Hospitals with standard methods of venous reporting. In addition we also evaluate the effectiveness of some embolic agents in treating pelvic venous insufficiency (PVI) through close follow up (12 months) using Visual Analogue Scale (VAS). Assessing the Growth Rate of Abdominal Aortic Aneurysms Greater than 5 cm
Introduction: There is very little contemporary data available on patients with large abdominal aortic aneurysm (AAA) to predict the rupture rate. The most commonly used predictor of rupture is the maximum AAA diameter. The growth rate of small aneurysms is well documented in the literature, in part due to surveillance and screening programs. Guidelines state that aneurysms should be repaired electively at 5.5cm for males. At this point aneurysms are generally either repaired or a decision is made that the patient is not a candidate for intervention and surveillance is discontinued. (Source: EJVES Extra)

The Impact of Ruptured Abdominal Aortic Aneurysm Diameter on Mortality from the ICVR
Introduction: The mortality following ruptured abdominal aortic aneurysm (rAAA) repair is high despite improvements in peri-operative care, centralisation of emergency vascular surgical services, and the introduction of Endovascular Aneurysm Repair (EVAR). The diameter of intact AAA has been shown to be a predictor of short and long term survival. The aim of this study was to analyse the impact of AAA diameter at presentation on mortality for ruptured AAA repair using contemporary data collected by the International Consortium of Vascular Registries (ICVR) from seven countries. SVS/AAVS Grading for Pulmonary Status and Contemporary Outcomes of Elective Endovascular Aortic Aneurysm Repair
Introduction: Pulmonary disease, in particular chronic obstructive pulmonary disease (COPD), is a known risk factor for increased postoperative morbidity and mortality. SVS/AAVS has proposed a grading system for pulmonary status including symptoms, radiographic changes, and pulmonary function tests. Our aim was to report how these grades influence outcomes after endovascular aortic aneurysm repair. (Source: EJVES Extra)
