Ilio-Iliac Arteriovenous Fistula due to Aorto-Iliac Aneurysm Rupture

A 73 year old male patient with history of ischaemic cardiopathy and diabetic nephropathy presented with acute left lower limb oedema and dyspnoea. Venous ultrasound found a patent deep venous system, but retrograde pulsatile flow was noted in the left common femoral vein. A computed tomography (CT) scan diagnosed a 56 mm infrarenal abdominal aortic aneurysm and a 44 mm right common iliac aneurysm (CIA) with a fistula to the left common iliac vein. He was treated with a bifurcated endograft landing on the right external iliac and left common iliac arteries, with previous embolisation of the right internal iliac artery.
