Off the Shelf Thoracic Endovascular Aortic Repair with Sandwich Technique for the Treatment of a Thoraco-abdominal Penetrating Aortic Ulcer
Penetrating aortic ulcers (PAUs) are the rarest subset of acute aortic syndromes, and a thoraco-abdominal (TA) location is uncommon. Endovascular surgery is considered first line treatment. Custom made branched/fenestrated endografts have been successfully applied in this disease but are unavailable in urgent settings. Off the shelf solutions may be required in high risk patients. The authors describe a case of a symptomatic rapidly expanding TA-PAU without a distal seal zone that underwent urgent endovascular repair. (Source: EJVES Extra)

Endovascular Repair for Ascending Aortic Graft Side Branch Pseudoaneurysm: a Report of Two Cases
A pseudoaneurysm arising from the side branch of the prosthesis, following ascending aortic replacement, is extremely rare. Re-intervention usually involves open surgery, replacement of the ascending aorta, or ligation of the side branch. I've Got You Under My Skin: Rare Presentation of a Ruptured Thoraco-abdominal Aortic Aneurysm
A 77 year old woman noticed throbbing between the left dorsal ribs (eighth intercostal space). Computed tomography angiography showed a thoraco-abdominal aortic aneurysm (TAAA; maximum diameter, descending thoracic aorta [DTA] 7.7 cm) with contained rupture into the subcutaneous tissues (See Fig. 1A) and an ascending aortic aneurysm (6.1 cm). The haemodynamically stable patient was treated by left carotid to subclavian bypass and, three days later, thoracic endovascular aortic repair with coverage and endovascular occlusion of the proximal left subclavian artery (See Fig. (Source: EJVES Extra)

Percutaneous thrombin injection: an alternative therapy for iatrogenic carotid artery pseudoaneurysms
Carotid artery pseudoaneurysm is a rare but potentially morbid complication of central venous catheter insertion. Traditionally managed by open surgical or endovascular repair, this article describes a case that uses percutaneous thrombin injection. Ruptured Pancreaticoduodenal Artery Aneurysm with Median Arcuate Ligament Compression: A Two Staged Approach to Management
Pancreaticoduodenal artery (PDA) aneurysms represent a small portion of rare visceral artery aneurysms. Rupture of these aneurysms results in fatal haemorrhage in up to 50% of cases, necessitating prompt endovascular or open intervention. As highlighted by a recent retrospective review, median arcuate ligament (MAL) release is an important part of management when these aneurysms are diagnosed in conjunction with median arcuate ligament compression (MALC).
