Ad Interim Results of a Multicentre Non-Randomised Study on Unibody Endograft Aortic Aneurysm Repair Using AFX ®2 for Less Invasive and Faster Endovascular Aortic Repair (AFX2-LIVE)
Objectives: The aim of this study was to evaluate intra-operative, peri-operative, and postoperative results in patients electively treated with the AFX ®2 anatomically fixed endografts for the endovascular repair (EVAR) of abdominal aortic aneurysms (AAAs) in a large cohort of consecutive patients (International Registered Report Identifier: PRR1-10.2196/16959). Moreover, the following secondary endpoints were addressed: operative time; intra-oper ative radiation exposure; contrast medium use; AAA sac shrinkage at the 12 month and five year follow ups; and any potential role of patients’ baseline characteristics and de...

Acute Surgery versus Anticoagulation for the Treatment of Superficial Vein Thrombosis in Patients with Varicose Veins: A Prospective Study
Introduction: Superficial vein thrombosis (SVT) is a typical complication of varicose veins of lower limbs. Although current guidelines recommend against acute superficial venous intervention in the case of SVT, a significant number of surgeries are still performed in many centres, especially when thrombosis extends to the junction with the deep veins. The aim of our study was to assess the outcomes of acute surgery, compared to anticoagulation, for the treatment of SVT. (Source: EJVES Extra)

Endovascular Exclusion of Juxtarenal Aortic Aneurysm in Concomitant Presence of an Aortocaval Fistula
Introduction: Aortocaval fistula is a rare vascular complication in which the aneurysmal sac erodes into the inferior vena cava. Open aneurysmal repair is associated with high mortality and morbidity rates. Management with standard endovascular aneurysm repair (EVAR) is not always feasible owing to anatomical restrictions. It is also associated with the development of persistent low flow type II endoleaks. (Source: EJVES Extra)

Acute Limb Ischaemia: Charlson Comorbidity Index Adjusted to Age, Biomarkers, and Prognosis
Introduction: Charlson Comorbidity Index adjusted (CCIa) for age is commonly used in outcome studies to adjust for patients ’ comorbid conditions and has been proven to be a good predictor of amputation rate and survival in patients presenting with acute embolic lower limb ischaemia (AELLI). Although biomarkers could potentially help to predict which patients will have unsuccessful limb salvage attempts or who will hav e poor functional outcomes after salvage, there are no studies that support its routine use in predicting limb salvage and survival after AELLI. Covered Stents versus Bare Metal Stents in Kissing Reconstructions of the Aortic Bifurcation
Introduction: Although endovascular therapy and primary stenting are now accepted as the gold standard for the treatment of aorto-iliac occlusive disease, few studies have compared covered stents (CS) to bare metal stents (BMS) in kissing reconstructions of the aortic bifurcation. The aim of our multicentre study was to analyse the short and mid-term results, by comparing CS to BMS in this location. (Source: EJVES Extra)

Comparison of Transverse versus Longitudinal Incisions for Femoral Endarterectomy and Patchplasty
Introduction: Access to the femoral artery for a femoral endarterectomy and patchplasty can be undertaken either through transverse (TI) or longitudinal incision (LI). LIs have been shown in previous studies to have higher groin complications, although these were undertaken in multiple types of vascular procedure. We looked at wound complications for patients undergoing common femoral artery endarterectomy (CFE) procedures only with or without plasty via TI or LI. (Source: EJVES Extra)

Technical Feasibility of Aortic Wall Calcium and Peri-Aortic Fat Tissue Quantification Using CT Angiography Images
Background: The prognostic value of aortic wall calcification (AWC), as well as peri-aortic fat tissue (PFT) has raised interest among researchers, but both methods have been applied to unenhanced computed tomography (CT) images. As patients with abdominal aortic aneurysms (AAAs) are invariably examined with CT angiographies (CTA), a method is needed that can be directly applied to contrast enhanced CT images. When Fast Fashion Doesn't Work One Needs a Tailor: Total Endovascular Repair by Custom Made Endograft of an Aortic Arch Aneurysm Associated with Bovine Arch
Introduction: Aortic arch aneurysm are rarely encountered in isolation; they are often associated with descending aortic dissections, aneurysms, or intramural haematoma (IMH). The growth rate ranges from 0 to 16 mm per year, with a higher risk of rupture as the aortic diameter and growth rate increase. A consensus has been reached, with an indication to treat isolated aortic arch aneurysms with a diameter> 55 mm. Treatment options include open surgery, endovascular repair, and hybrid techniques. Haemodynamic Implications of STABILISE Technique for Aortic Dissection Repair
Introduction: With uncertainty about surgical management of aortic dissection, the STABILISE technique has shown favourable aortic remodelling outcomes at 12 months. While awaiting longer term data, computational modelling can be a way of non-invasively obtaining haemodynamic effects, such as pressure and wall shear stress, of the technique in patients. Particular areas of interest are: (i) the effect of intimal disruption and relamination; and (ii) the effect of the bare metal stent in the visceral aortic segment. (Source: EJVES Extra)

Primary and Assisted Patency of Vascular Access in End Stage Renal Disease Patients at One Year: An Audit of All Vascular Access Procedures
Introduction: Vascular access is still considered the ‘Achilles heel’ of haemodialysis. It is important to analyse the current practice outcomes, including patency rates and timing of formation of access in the predialysis population, as these are important parameters for the management of the vascular access pathway in patients with end stage rena l disease (ESRD). A New Standard of Care for Dialysis Access? Achieving High Usability and Long Term Patency Creating Rates Arteriovenous Fistula with External Support: A Large Retrospective Review
Introduction: Since 2018, creating arteriovenous fistulae (AVFs) with an external support device has become the standard of care (SoC) in our centre, as it has demonstrated beneficial patient outcomes at six months in multiple small clinical studies. Here, we report two year AVF outcomes with external support in the largest patient cohort to date for radiocephalic AVFs (RCAVFs). (Source: EJVES Extra)

A Fast-Track Protocol for Carotid Artery Surgery
Introduction: Indications for carotid endarterectomy (CEA) are well defined, but different approaches are proposed by distinct surgical schools. Nonetheless, a general consensus exists about a traditional protocol consisting of general anaesthesia with cerebral oximetry monitoring, longitudinal incision with patch closure, postoperative laterocervical drainage and fasting, and at least two days of postoperative observation. In the last few years, fast track protocols have been applied widely and successfully in many surgical fields; however, they are essentially lacking for carotid surgery. (Source: EJVES Extra)

Pseudoaneurysm Ligations in Intravenous Drug Users: A Single Centre Analysis of Bed Days Lost to Further Postligation Procedures
Introduction: Pseudoaneurysms caused by illicit intravenous drug use (IVDU) are a relatively common reason for admissions across vascular centres. Unfortunately, people who inject drugs (PWID) put a disproportionately large burden of care on an already strained health service and particularly on scarce tertiary centre beds. Pseudoaneurysms secondary to IVDU are a vascular emergency, with a high incidence of morbidity and mortality. They are commonly treated surgically with arterial ligations, a procedure bearing a significant chance of future complications such as limb loss. (Source: EJVES Extra)
