Caries Management in Special Care Dentistry
The objective of this article is to provide a summary of the current evidence-based recommendations for caries management in patients with special health care needs (SHCNs). Considerations regarding caries risk assessment and preventive measures are also discussed with the goal of helping clinicians to manage the caries disease process using a person-centered approach and risk-based interventions. Importantly, most of the evidence is still based on the general population, because the evidence for those with SHCNs is still limited. (Source: Dental Clinics of North America)

Teledentistry for Patient-centered Screening and Assessment
Teledentistry is a powerful tool for connecting oral health providers with patients who cannot easily visit a dental office, such as patients with special health care needs. Teledentistry is a skill that must be learned and this article will review key concepts that will allow providers to be better prepared to use it within their practices. These concepts include considerations for data collection and information that is necessary for a successful teledentistry visit. The authors also provide different examples of teledentistry in action, such as guided oral hygiene or dental screenings. Lastly, the authors review some un...

The Prenatal Diagnosis and Consultation for Cleft Lip and Palate Prior to Presurgical Infant Orthopedics and Early Dental Care.
Children born with cleft lip and/or cleft palate (CL/P) have unique treatment needs and benefit from a team of health care providers, including surgeons, dentists, and allied health professionals. Presurgical infant orthopedic appliances can reduce the severity of the birth defect and can be provided to patients by a dental health care professional starting at birth. The dental needs of patients with CL/P are multifaceted and having an established dental home to monitor growth and development and help with disease prevention is key to improve oral health. Now Is the Time for Special Care Dentistry
I am excited to share this emerging issue on “Special Care Dentistry” and make an honorable note of thanks and admiration to my predecessor and late editor, Dr Burton S. Wasserman, who paved the way with two issues in 2009 and 2016, on this important area of the oral health profession. It is with much enthusiasm these topics were proposed and vetted with the guidance of Mr John Vassallo, the editor of Dental Clinics of North America, and Ms Ann Gielou Posedio, the developmental editor, during what the profession will note to be one of the most catalyzing and evolving periods of our profession. Thinking Outside the Tooth
This article highlights and discusses how to take a complete and systematic pain history and the important red flags to recognize in patients presenting with perplexing nonodontogenic orofacial pain. Cause and epidemiology, clinical presentation, clinical evaluation and diagnosis, and management options for common neuropathic pain conditions are included. Neuralgia and neuropathic pain conditions and red flags as secondary cause of orofacial pain are more common in older-aged patients. (Source: Dental Clinics of North America)

The Impact of COVID-19 on the Oral Health of Patients with Special Needs
In March 2020, WHO declared a global public health emergency due to the spread of the Coronavirus Disease 2019 (COVID-19). As one of the societal responses to the emergency, medical and dental elective care was suspended, disproportionally affecting persons with special needs. As many of the special needs population live in a communal environment, they were at higher risk of being infected with and dying from COVID-19. Consequently, their access to medical and dental services was limited to emergency care. A method of reaching these populations evolved by the expansion of telehealth, including dentistry, to provide diagnos...

Soft Tissue Regeneration at Natural Teeth
This article provides an overview of the best-documented surgical techniques for recession coverage and draws conclusions for the clinician. Use of a connective tissue graft with either coronally advanced flap (CAF) or tunnel is the most predictable technique for the treatment of single and multiple gingival recessions. Long-term results exist only for CAF with/without connective tissue graft providing evidence for long-term stability with only minor relapses. Soft tissue replacement materials and biologics may represent a valuable modality to additionally improve the clinical outcomes obtained with CAF alone or, in certai...

Regenerative Periodontal Therapy in Intrabony Defects and Long-Term Tooth Prognosis
In this chapter, the results from a relatively recently performed systematic appraisal of the literature on the long-term outcome of regenerative periodontal treatment in intrabony defects are presented. Periodontal regenerative procedures in intrabony defects yield significantly better clinical outcomes compared with conventional surgery and result in high rates of tooth retention on a medium- to long-term basis. Combination approaches seem, in general, more efficacious compared with monotherapy. (Source: Dental Clinics of North America)

Introduction
Periodontal regeneration aims to retain teeth while restoring the attachment between the dental and periodontal structures. This goal is highly achievable with the emerging technologies and a deeper understanding of biology, material science, cell function, and molecular regulation of tissue responses. A holistic approach is needed to connect these knowledge under an innovative perspective to accomplish the regeneration of periodontal tissues. (Source: Dental Clinics of North America)

Inflammation and Periodontal Regeneration
Technological innovations in cellular and molecular aspects of tissue engineering ‐‐scaffolds, stem cells and 3D printed tissues ‐‐have been dramatically increased in the last decade. However, regenerative treatment still has challenges in translation to clinic. This is partly due to failure of addressing an essential element of wound healing, inflammation. It is now well -recognized that inflammation is an active process. This paradigm shift opened up a new avenue of therapeutic approaches called “host-modulation.” Host-modulation therapies capable of modulating inflammatory response at multiple levels and mim...
