Systemic Factors in Temporomandibular Disorder Pain
The science of temporomandibular disorder (TMD) pain and its management has gone through significant changes during the last several decades. The authors strongly feel that the effect of systemic factors influencing TMD pain has been largely overlooked and poorly accounted for, even in established pain-management programs and protocols. The hope is that this article will act as a wake-up call for the pain management community to consider the importance of adequate knowledge of the systemic factors that affect the experience of TMD pain by the patient. Temporomandibular Joint Disorder Comorbidities
Comorbidity is a distinct additional condition that either existed or exists during the clinical course of a patient afflicted by the condition/entity in question. The clinician attempting to manage temporomandibular joint disorder (TMD) and TMD pain must realize that recognition and management of the comorbidities are essential to the successful management of the same with optimal pain control. When TMD presents with multiple comorbidities, the task for the clinician becomes more complex. It is the hope of the authors that this condensed version of TMD-associated comorbidities acts as a primer for understanding the signif...

Temporomandibular Joint Imaging
Prescriptions for imaging studies for temporomandibular disorders are based on the patient ’s complaint, history, and clinical findings. Appropriate selection criteria and justification for imaging examinations must be followed. Because the temporomandibular joint is composed of both hard and soft tissues, different studies are prescribed based on the clinically suspected condition. Cur rent imaging modalities include panoramic radiographs, cone-beam computed tomography, and MRI. The entire examination must be interpreted, and the findings recorded in the patient record. No one imaging modalities is suitable for all pati...

Temporomandibular Disorders and Dental Occlusion
Throughout the recorded history in the literature of temporomandibular disorders (TMD) there have been a variety of opinions as to its primary cause. Those supporting an occlusal basis of TMD opined that occlusal dysfunction is either the primary cause for or a significant etiopathogenic factor in the causation of TMD. Most of the current literature, however, points to evidence in another direction and questions the causal role of occlusion and occlusal disharmony in TMD etiopathogenesis. Recognition of this evidence-based literature is paramount in eliminating and preventing the chances of overtreatment of patients with T...

Temporomandibular Joint
Temporomandibular joints (TMJ) are one of the most complex joints. Each one is located on one side of the face, and are composed of mandibular fossa, joint tubercle, and condylar process of mandible, separated by an articular disk. To these structures are attached ligaments and muscles, which will provide stability and movement. When TMJs work properly, jaw movements can be performed without pain or discomfort. It is important to mention that the complex formed by both TMJs will confront the maxillary with the mandibular bone and therefore will be related to the occlusion, linking these structures during growth and develop...

Classification and Diagnosis of Temporomandibular Disorders and  Temporomandibular Disorder Pain
Designing classification systems and developing diagnostic criteria for temporomandibular disorders is difficult. An appreciation of the utility and applicability of these entities requires an understanding of the importance of each, the differences between the two, and how they may be optimally operationalized for both clinical and research activities in light of their inherent advantages and limitations. In addition, consideration for adopting newer approaches, such as following ontological and precision-based medicine principles, accounting for genetics/epigenetic and neurobiological factors, and the inclusion of biomar...

Myofascial Temporomandibular Disorders at a Turning Point
Unvalidated theories have been proposed for the etiopathogenesis of masticatory myofascial temporomandibular disorders (mTMD). Modalities such as cone-beam computed tomography/computed tomography and MRI contributes little to the diagnosis of mTMD. Diagnosing mTMD is based on the recognition of “familiar pain” presentation in the masticatory myofascial tissue. This assessment tool contributes little our understanding of the underlying disease process. Thus, management of mTMD is empirical and arbitrary. Exploring emerging technologies to identify biomarkers and objectively assess myofa scial tissue physiology in diseas...

Temporomandibular Disorders Within the Context of Sleep Disorders
This paper provided an overview of the knowledge on the relationship between temporomandibular disorders (TMDs) and the main sleep conditions and disorders of dental interest, namely, sleep bruxism (SB), sleep apnea, and gastroesophageal reflux disease (GERD). It emerged that although the topic of SB as a possible detrimental factor for the stomatognathic structures has been the most studied, evidence is growing that SB, obstructive sleep apnea, and GERD, all belong to a circle of mutually interacting sleep disorders and conditions that, in turn, may be associated with TMDs. The pathophysiology of the cause-and-effect rela...

Surgical Treatment of Temporomandibular Joint Derangement
The improvement in diagnostic accuracy, improvement of the endoscopic equipment, better selection of patients for open TMJ surgery, and increased focus on research and education are promising for the treatment of the group of patients with TMJ derangements. In the future, prospective randomized clinical trials need to be performed to give the clinician guidelines as to which type of intervention should be chosen in a particular patient base on accepted criteria for diagnosis and treatment of TMJ derangement. Challenges for the Dentist in Managing Orofacial Pain
This presentation describes a patient ’s extensive and expensive search for relief of pain in the orofacial area. The journey includes many diagnostic errors and failed and likely unnecessary treatments. A systematic approach to problem definition and rule-out reasoning for the differential diagnoses based on the International Classif ication of Orofacial Pain is described. Conservative treatment was implemented with satisfactory results. Common pitfalls in the management and treatment of complex pain patients are discussed. (Source: Dental Clinics of North America)
