[Therapeutic options for stress urinary incontinence following surgery for benign prostate enlargement].
Abstract Persistent stress urinary incontinence following surgical treatment of bladder outlet obstruction based on benign prostate enlargement (BPE) occurs at an incidence of approximately 2.2 %, although incidence rates vary significantly between different surgical options. Current evidence on SUI treatment of BPE patients is usually based on postprostatectomy incontinence patient cohorts or small subgroup analyses of greater patient collectives. Studies that focus on SUI after surgical treatment of BPE are still rare. Based on the current low-level evidence, it can be assumed that pelvic floor muscle training...

[Novel minimally invasive treatment options for male lower urinary tract symptoms].
Abstract The main objective of novel minimally invasive treatment options is to be equally effective to current reference techniques while providing a superior safety profile. Especially the preservation of sexual function, including ejaculatory function, is of great interest to patients facing surgical treatment of lower urinary tract symptoms (LUTS). Ideally, a truly minimally invasive procedure can be performed under local anaesthesia in an outpatient setting. Among emerging medical devices, the prostatic urethral lift (PUL; Urolift) demonstrated rapid and durable relief of LUTS in selected patients without a p...

[Myocardial infarction in a young patient with seminoma during chemotherapy with cisplatinum, etoposide, and bleomycin].
mann KP Abstract INTRODUCTION:  More than 90 % of all patients with testicular germ cell tumours can be cured effectively. The mainstay of treatment is chemotherapy with cisplatin, etoposide and bleomycin (PEB). This regimen is usually well tolerated and does not lead to serious adverse events. Cardiovascular complications are encountered very rarely, but have gained increasing attention in recent years. CASE DESCRIPTION:  A 33-year-old man with a testicular seminoma, clinical stage 2b, was subjected to PEB chemotherapy. At the end of the first treatment course, he had an acute ST-elevation myocar...

[Differences between the inpatient complication rate after ureterorenoscopy and the 30-day outcome reported by the patient - results of the German prospective BUSTER study].
Abstract BACKGROUND: Following ureterorenoscopic stone removal (URS), patients are generally discharged after a short hospital stay, so that the estimation of complication rates is based on a narrow timeframe. Data derived from the so-called BUSTER-project (Evaluation of ureterorenoscopic stone management - results in terms of complications, quality of life and stone-free rates) were therefore analysed with respect to complication rates during hospital stay and patient-reported 30-day-complication rates and then correlated with quality of life (QoL) data. METHODS: Data of 307 patients undergoing URS were ...

[Immunotherapy with checkpoint inhibitors in local advanced and metastatic urothelial carcinoma].
Abstract The approval of immune checkpoint inhibitors has started a new era in the treatment of urothelial carcinoma, which, so far, has been a disease with limited treatment options. However, in the current German S3 guideline for bladder cancer the remarkable improvements provided by immune checkpoint inhibitors have not been reflected. As cancer immunotherapy is a novel approach, there are several questions related to the daily clinical routine: What clinical data was used as the basis for the approval of the individual substances? What does this mean for previous treatment algorithms? What issues must be consi...

[The role of biomarkers in the diagnosis, prognosis and therapy of patients with immuno-oncological treatments of genito-urinary carcinomas].
not J Abstract In recent years, fundamental research has yielded new insights into tumour biology, and new treatments have been developed. This review highlights the role of the pathologist and how he can support clinicians to find the right treatment for each patient. We explain the problems of the molecular subgroups of bladder cancer, the role of neo-adjuvant chemotherapy in the context of these findings, and show the important role of checkpoint inhibitors. Furthermore we focus on kidney cancer, with the clear cell carcinoma as the most frequent tumour type. We briefly consider prostate cancer, which as a horm...

[Immune checkpoint inhibition in metastatic urothelial carcinoma].
l A Abstract The treatment of metastatic urothelial carcinoma is characterized by the administration of combinatory regimes of chemotherapy. In the first-line treatment theses regimes demonstrate relatively good objective response rates and prolongation of overall survival. The prognosis declines if the patient is refractory to platin in the second- or third-line treatment or if the patient is ineligible for cisplatin in the first-line setting. Cisplatin ineligibility is defined by a poor renal function or subsequent comorbidities. The new class of immune checkpoint inhibitors (ICI) has led to an impressive improv...

[Immunotherapy for renal cell carcinoma - current status].
Abstract Systemic treatment of metastatic renal cell carcinoma (mRCC) has substantially changed during the last 2 years due to approval of the immune-checkpoint inhibitor Nivolumab (Opdivo®) and new multikinase inhibitors (Cabozantinib, Lenvatinib, Tivozanib). The german kidney tumor guideline strongly recommends Nivolumab and Cabozantinib as 2nd line treatments after prior VEGF targeted therapy. CheckMate 025, the prospective randomized trial which led to approval of Nivolumab demonstrated improved overall survival (26 month vs. 19.7 month; hazard ratio 0.73; p = 0.0006) and response rate (26 % vs. 5 ...

[Severe thoracic bleeding - 25 years after nephrectomy].
We report a case of a 62 year old patient, who was hospitalised due to haemorrhagic shock. The cause was a rupture of the pulmonary artery due to metastasis of a renal cell carcinoma. This cause is now reported for the first time. The primary tumour (pT2,pN0,M0,R0) was resected 25 years ago. This case demonstrates that after curative nephrectomy pulmonary metastasis can occur, even after decades. This is why patients of this type need lifelong follow-up. This article focuses on female stress incontinence in the form of pelvic floor dysfunction and urethral sphincter deficiency, although isolated stress incontinence accounts for less than half of all incontinence cases. Especially in women of old age and those with neurological comorbidities, the causes of incontinence are mostly multifactorial. Also it has to be considered that the female bladder, urethra, uterus and rectum as well as the muscular and ligamentous structures of the female pelvis minor are affected by phases of fertility, possible pregnancies, births and menopause in addition to the normal ageing process. ...

Abstract Endometriosis is the second most common benign female genital disease after uterine myoma. This review discusses the management of individual patients. This should take into account the severity of the disease and whether the patient desires to have children. Particular emphasis is laid on the anatomical intersections which, when injured, can lead to persistent damage of the anterior, middle or posterior compartment and are not infrequently the cause of urological and urogynaecological follow-up measures. Typical symptoms of endometriosis include chronic pelvic pain, subfertility, dysmenorrhoea, deep dysp...
