[Current Developments in the Medical Treatment of Advanced Urothelial Carcinoma of the Bladder.]
This article focuses on important clinical trials and developments of the last two years, with a review of literature and proceedings of the relevant meetings (ASCO, GU-ASCO, EAU, AUA). The general benefit of perioperative chemotherapy is confirmed. Chemotherapy with Gemcitabin/Cisplatin remains the therapy of choice in the palliative setting. So far, targeted therapies did not show convincing results. PMID: 24500963 [PubMed - as supplied by publisher] (Source: Aktuelle Urologie)

[Nocturia - an often misjudged problem].
Abstract Nocturia - waking up during the night due to the urge to urinate and empty the bladder - is a serious problem for affected patients. In the past decades, nocturia has been primarily regarded as an irritative symptom of benign prostate hyperplasia (BPH). This symptom is however frequently not influenced by different BPH treatments. In the last couple of years one has come to the conclusion that the prostate is less involved and in part responsible for the symptoms since women are also frequently affected. For these reasons nocturia is looked at differently. It is a highly prevalent symptom which neither qu...

[Diagnosis and management of the undescended testis - an update in the light of the current guidelines].
Abstract Cryptorchidism, or undescended testis (UDT), occurs in 1-3% of male term infant births. At least two-thirds of UDTs will descend spontaneously, typically during the first 6 months of life. UDTs are associated with loss of spermatogenic potential and testicular malignancy in the long term. Orchiopexy performed prior to puberty may significantly reduce the malignant potential by up to 4-fold. Neoadjuvant hormonal therapy starting at 6 months of life has been shown to potentially improve the testicle's fertility index and should be part of the therapeutic concept. However, the use of hormonal treatment and H...

[Utility of the Serum CRP Value for Assessing the Prognosis and Therapeutic Response of Urological Malignancies].
s S Abstract C-reactive protein (CRP) is an unspecific marker of systemic inflammation. It is known to be elevated in autoimmune disease, traumata and malignancies. Increased CRP levels have specifically been shown to be associated with disease progression and prognosis in various studies on renal cell carcinoma and transitional cell carcinoma. Although CRP, unlike PSA, is neither organ-specific nor tumour-specific, studies were able to show that increased CRP values are an independent prognostic marker for tumour-specific survival of patients with prostate cancer. In metastatic and castration-resistant prostate c...

[Stone treatment after urosepsis in elderly or multimorbid patients - influence on sepsis recurrence rate and rehospitalisation].
Abstract In elderly and multimorbid pa-tients urosepsis is a particularly serious condition with a high mortality. Whether a secondary therapy for an underlying stone disease after -initial sepsis treatment improves the prognosis of these patients has not been systematically investigated. We retrospectively reviewed the clinical course of elderly and multimorbid patients with urosepsis regarding the frequency of rhospitalisation and sepsis recurrence after initial and subsequent therapy.Patients with and without urolithiasis and urosepsis were compared. Inclusion criteria were multimorbidity, defined by an ASA sco...

[Penile cancer - a challenge for modern uro-oncology].
Abstract Due to the low incidence of penile cancer in Europe and North America and often decentralised patient management, data on the treatment of penile cancer are very rare. Nevertheless, recently relevant data have become available for the improvement of the therapy especially for the primary tumours. Penile conserving surgery is becoming more important for early tumour stages. The management of lymph nodes plays a key role for the successful treatment of patients with penile cancer. An invasive lymph node staging is recommended for all patients with primary pT1G2 or higher tumours. Neoadjuvant chemotherapy fo...

[Does the Surgeon have an Influence on Clinical Voiding Parameters and Postoperative Sexual Function after TURP?].
In this study we investigated the possible influence of individual surgeons on clinical voiding parameters and post-operative sexual function after TURP.We analyzed 123 patients with a mean age of 70 years (46-88 years) who had undergone TURP secondary to BPS. To evaluate the clinical parameters, patients completed the IPSS and IIEF-15 questionnaires preoperatively and 1 year postoperatively. Additional parameters including urinary flow and post voiding residual volume were also collected.A statistically significant improvement in voiding function was noted postoperatively. The mean IPSS improved from 19.35 to 6.49 (p

[Carcinoma of the Pararenal Gland Infiltrating the Surrounding Tissue and Involving the Inferior Vena cava].
Abstract A 61-year-old woman presented with a 2-month-history of progressive deterioration, increasing exertional dyspnoea and pain in the right upper abdomen (past medical history: bronchial asthma and hypertension). The physical examination showed mild generalized weakness, tenderness in the right upper abdomen, and ascites.Laboratory studies did not reveal any hormonal abnormalities. A CT angiogram revealed a mass of the right adrenal gland with distinct invasion into the inferior vena cava, and tumour thrombosis that extended proximally into the right atrium. Distally, the tumour ended at the caudate lobe of t...

[Medical reviews of claims for damages and errors in urology].
Abstract In liability law, a medical review is considered to be an expert opinion that is provided at the request of those involved (patient or physician) of a course of treatment. It must be carried out according to defined criteria with the aim of providing a plausible basis for arbitration. This is achieved by means of an objective determination of the facts, a reasonable assessment of the error(s) and (where appropriate) a realistic description of the injury involved. The following fields should be covered in a concise review: conditions and procedures applied to the review, criteria to be used in the review, ...

Malignant Bladder Pheochromocytoma with SDHB Genetic Mutation.
Abstract A 30-year-old man presented with micturition pain and was diagnosed with a submucosal tumor in the right wall of the bladder with metastasis to the right obturator lymph node. Transurethral resection led to a diagnosis of invasive malignant pheochromocytoma. Radical cystectomy, neobladder reconstruction and bilateral iliac lymph node dissection were performed. Genetic analysis revealed succinate dehydrogenase B-associated hereditary pheochromocytoma/paraganglioma syndrome. 10 months after the operation, he had no evidence of recurrence. [Geriatric Urology: Special Features of Palliative Long-Term Urinary Bladder Drainage in the Context of the Geriatric I's].
n I Abstract In situations at the interface of urology and geriatric medicine the palliative management of geriatric patients with urinary incontinence of voiding dysfunction occupies a central position. Influencing factors that need to be considered by the surgically active urologist are not only the high age and the thus associated vulnerability of the geriatric patient but also other aspects such as the patient's wishes, the expectations of care-giving relatives and the sexual situation. In this light a strict and individually adapted indication for placing a palliative indwelling catheter is mandatory. First o...

[Impact of Operator Experience on TURB of High-Grade Non-Muscle-Invasive Bladder Cancer - Analysis of 254 Second Resections at a University Teaching Hospital].
Roth S Abstract In departments with urological training of residents, part of the TURB procedures are performed as "teaching surgery". Does resection quality and early recurrence depend on the operator's experience? From July 2007 to February 2012 254 second resections (TURB) after Ta high-grade and T1 high-grade bladder tumours were performed at our institution. The surgeons were stratified into "junior residents" (first and second year of training), "experienced residents" (3rd-5th year of training), board certified urologists, consultants and chief surgeons. We analysed the risk of...
