[Palliative care in an outpatient urological setting].
er J Abstract In their daily routine, urologists in general and uro-oncologists in particular are often confronted with non-urologic complications. To meet the need for measures of symptom control at the interface between genuine urologic and general palliative care, it is necessary for the partners from special outpatient palliative care (SAPV), general outpatient palliative care (AAPV), medical centres and other service units to cooperate. There are a significant number of urological practitioners who have completed trainings in palliative care or passed an exam as 'qualified palliative physician (QPA)'. Dependi...

[The significance of psycho-oncology for urology].
Abstract National and international guidelines and certification criteria for comprehensive cancer centres require the psycho-oncological co-treatment of distressed cancer patients. Of course, this also applies to the field of uro-oncology. The individual need for psychosocial support should be systematically evaluated by "distress screening" procedures. While in general about 30 % of all cancer patients are considered severely distressed, this rate seems to be lower (about 20 %) in prostate cancer patients, the largest uro-oncological entity. Many patients find it difficult to accept psychotherapeut...

[Facets of doctor-patient conversations in urological practice: recommendations for intervention from psycho-oncology].
This article examines facets of doctor-patient interaction from a psycho-oncological perspective. These include risk perception, lay theories, lifestyle changes, sexuality and, most importantly, dealing with anxiety. Based on recent research results, suggestions are given for the communication with patients. However, the article also shows the limits of physician-patient conversations. Professional psychological patient support is required if the cancer causes a psychological crisis or if deeper problems are reactivated, or if some problems simply require more time and rest than is available in everyday practice. Therefore...

[The ISAR-positive patient in urology].
CONCLUSION:  The ISAR-screening identifies the "geriatric" patient in urology, who is threatened by his vulnerability, chronification, loss of autonomy and multimorbidity when being hospitalised. The regular implementation of the systemic approach of applying ISAR-screening and the further assessments into the clinical daily routines of urology constitute the major challenge in the future. [Current trends in flexible ureteroscopy: an update].
ernik A Abstract Flexible ureteroscopy (fURS) has become a success story in urology. Since its implementation into the treatment of urolithiasis, fURS has demonstrated excellent clinical performance and safety. In recent years, the spread of disposable ureteroscopes has shaped the field of endoscopic stone treatment. However, the primary advantage of these devices is that they improve the workflow in urology theatres (no sterilisation, no repair costs) and possibly minimise bacteria transfer in working channels rather than being a real technological "game changer". Novel disruptive improvements such as a...

CONCLUSION:  The newly designed miniaturised MIP sets maintain the favourable pressure features of the earlier 17.5 MIP sheath. Although the diameter has been reduced to 12F or 9.5 F, the intrapelvic pressures remained below 40 cmH2O when regular irrigation settings by gravitation or pump irrigation were used. Access to knowledge, data collection and communication are thus simplified and accelerated. A wide range of health apps exists for the public, patients, health professionals and medical education. In particular, in the field of urology, several high quality mHealth contents have been created in recent years. Nevertheless, there is still need for improvement and development. The ubiquity of health apps might also bring particular risks. Healthcare professionals can use their priv...

[Enhanced endoscopic imaging for urothelial cancer].
Abstract The precise identification and detection of bladder tumours is a prerequisite for complete transurethral resection and, thus, the reduction of recurrence. White-light cystoscopy remains the gold standard and has been further improved by the introduction of digital HD techniques. New digital techniques such as narrow-band imaging (NBI) or the IMAGE 1 S-System have been introduced to better visualise suspicious areas. They are particularly valuable in the outpatient setting. However, fluorescence or photodynamic diagnostics (PDD) after instillation of hexaminolevulinic acid into the bladder is the only appr...

CONCLUSIONS:  In contrast to ISH and the examined histological criteria, p16INK4a allows a reasonable prediction of HPV status. Neither HPV DNA nor its surrogate markers independently impacted CSM. As urethral tumour invasion does not independently influence CSM, the recent pT classification can be considered useful for prognosis. [The significance of palliative surgery in castration-resistant prostate cancer].
Abstract If prostate cancer recurs after primary treatment, deprivation therapy with LHRH analogues or antagonists is the treatment of choice in men with metastatic prostate cancer. However, this treatment only achieves palliative results. Median time to progression ranges between 11 and 78 months. After the introduction of Docetaxel as a first-line treatment in castration-resistant prostate cancer (CRPCA) and cabazitaxel as a second-line chemotherapy, several new drugs containing abiraterone, enzalutamide, radium 223 and sipuleucel-T have become available, which can lead to complete or partial remissions in metas...

[Palliative surgery for metastatic urothelial cancer].
SF Abstract Palliative surgery is performed in order to relieve symptoms as well as to increase survival in selected tumour entities. For urothelial cancer, its role is limited and clinical data are limited, too. Yet with better responses being achieved to systemic therapy, more and more surgical procedures are being carried out in the form of individualised treatment concepts and are then published as case series. For patients with low metastatic load, or if the tumour is restricted to lymph nodes inside and outside the pelvis, as well as for singular pulmonary metastasis, there seems to be a survival benefit in...

[Symptomatic renal tumours in metastatic renal cell carcinoma - surgical options].
Abstract Between 15 and 20 % of patients diagnosed with renal cell carcinoma suffer from metastatic disease by the time of diagnosis. In addition to systemic therapy, there are various surgical strategies to treat the primary tumour, e. g. cytoreductive nephrectomy (CN) or palliative nephrectomy (PN). In the immunotherapy era, the standard treatment was to perform CN followed by interferon therapy, based on 2 prospective randomised trials and their combined analysis. Since the introduction of targeted therapy around 2005, data of that quality has been missing and clinicians have had to rely on big retrospectiv...

[Symptomatic urinary obstruction in patients with metastatic malignancies].
Abstract Symptomatic urinary obstruction, characterised by decreased or absent urinary outflow, is a common concomitant disease in patients with urological malignancies or carcinomas forming abdominal metastases. While acute urinary obstruction is associated with renal colics, chronic urinary obstruction often goes unnoticed for a long time. In addition to intrinsic causes such as upper urinary tract tumours, this condition is often caused by external compression of the ureter by tumour masses. Placement of a ureteral JJ stent or a percutaneous nephrostomy tube results in early disobstruction. Depending on the pro...

[Nocturia in men with benign prostatic hyperplasia].
Abstract Nocturia, defined as nocturnal micturition with a frequency of at least once per night, is one of the most frequent lower urinary tract symptoms in men with benign prostatic hyperplasia (BPH) and often causes them to consult a physician. Nocturia is often bothersome and responsible for increased morbidity and mortality. Nocturia can be caused by increased fluid intake, increased diuresis or decreased bladder capacity, either alone or in combination. The underlying pathophysiology of nocturia can only be detected by methodical evaluation of the patient. Bladder diaries for 3 days are an essential part of t...

Abstract Pharmacological treatment of BPS is often considered the first therapeutic approach for patients who experience progression in lower urinary tract symptoms (LUTS).α1-adrenoceptor antagonists are often chosen as the first line pharmacological treatment, due to their rapid onset of action, as well as their significant impact on LUTS and Qmax and post-void residual. Adverse effects appear to be low. With larger prostate volume and LUTS, long term treatment with 5α-reductase inhibitors (5ARIs) significantly improves LUTS and post-void residual. In addition, 5ARIs have a significant impact on the re...
