So I've had a few VA patients lately who have been a bit unique in terms of medications and was hoping to get some input on people's experience with Cyproheptadine, as I have some questions about it that my supervising attending hasn't been able to answer (overall he's great, but just doesn't use Cyproheptadine and admits he doesn't really know a lot about it). Anyone else suffer from insomnia?
I realize that the nature of our field and being on call and woken up sporadically and randomly throughout the night (negatively) affects sleep cycles so even when I'm in the comfort of my own bed at home I don't sleep well; and then it just becomes a really vicious circle. I've tried all the "usual" remedies (melatonin, cherry juice, no screentime before bed, darkened room, meditation, etc.) and don't want to resort to meds at all. Any suggestions? Self-reflection after Quitting Medical School in 2015.
I'm 31 years old. My Xanax addiction started innocently when I was probably about 22, in 2012 -- in the prime of my life, at the time. I found myself struggling with a bit of insomnia as I was in the process of applying to medical school and taking Adderall in the name of vocational success. That's how Xanax entered my life: upon an initial and cursory complaint of difficulty sleeping. Just failed an Ochem exam.
Long vent incoming. Please disregard the ironic screenname. I usually score well above average on our weekly exams. This past week I became super motivated to do as well as I possibly can. I studied 34 hours since Monday a.m. just for ochem. I followed the study guide to a T. Studying became all that I do. I wasn't able to get through all the material as I was being so thorough. I did learn about 80% of the material completely. I developed insomnia and slept 2.5 hrs last night and 4hrs... Just failed an Ochem exam. (Source: Student Doctor Network)

Prescribing Propanolol for anxiety/insomnia with stimulants?
I have a teenage patient with a history of ADHD that I'm seeing who was prescribed low dose of Propanolol (20 mg) and Adderall XR 20 mg by his previous provider. He says he is only taking the Propanolol as needed for sleep but I worry about him getting unopposed Beta Blockade blockade if he takes the Adderall and Propanolol together. I know he is on a low dose of Propanolol but I wish to avoid any type of blood pressure fluctuations if possible especially with him taking it as needed. I... Why isn't Fisher Wallace Cranial Simulator and similar devices prescribed/discussed more?

Billing for Med management & Psychotherapy - 99214 + 90833
I see my follow-ups for 30-minutes, and I'm having a hard time seeing in my mind how I don't end up billing most of my follow-up visits as level 4 (99214) visits with a psychotherapy add-on code (90833). I am doing psychotherapy for more than 16 minutes with most of my patients, and if you look at the criteria for a level 4, it's hard not to hit that threshold. I know that taking a W in undergrad is not a big deal, but I'm wondering about a W in informal post-bacc? I decided to take OChem 2 with the lab over a 5 week summer semester. Needless to say, the pace is ****ing brutal. Due to the pace, and a really bad attack of insomnia (which I am seeing a doctor and psychologist for), I fell behind.
