Combined EM/Diagnostic Radiology?
Okey, why did i thought of this combination. I work as EM resident in europe and we do not evaluate much CT scans. You can learn self of course. But my thought is - if you are double specialized with rad maybe it can give much to patient flow, you can take quick evaluations self and avoid long with times till you get rad answer. You have good ultrasonography skills and if you want to avoid a bit patient contact can work for exemple 50/50 or 60/40. Maybe it is just me who thinks that way... Combined EM/Diagnostic Radiology? TAP block with neuropathic pain
Have new referral and wanted to see if anyone has seen this. Young guy with recent resection of sigmoid tumor. Had TAP block with US prior and did well for the first week or 2. Now has odd neuropathic pain in similar distribution you would expect coverage from block. CT scan and lab work up for infection done without cause. Has anyone run into this before? I did a quit lit search, but did not find anything on neuropathic pain post block. (Source: Student Doctor Network)

CT sufficient prior to Epidural?
Patient is an elderly female with h/o cardiac stent on anticoagulation also with a pacemaker that is not compatible with MRI. She has lower extremity radicular pain. Ordinarily I would like to have a MRI so that I can see soft tissue pathology. There is a CT scan which notes that there "may be some mild canal and neur foraminal narrowing." Clinically pain is only on ambulation with no associated neurologic deficits by history nor in the office on exam. Would you proceed with intervention... CT sufficient prior to Epidural? Another "My Worst Airway Case"
56 year old presented to the OR this week for a PEG and IV infusion port and the surgeon asked for sedation. She had a squamous cell tumor from the mandible to the base of the neck on the left side protruding 4" off the side of the neck and was 4 inches wide and 5 1/2 inches long. CT scan showed tumor protruding into the oral pharynx, involving the tongue, lingual and palatine tonsils on the left side and extending more than half way across the pharynx at the level just superior to the... Interesting Case
90yo AAF for nailing of femur fx. 80kg. 5'4. Severe dementia. Very Combative and pulls out IVs on the floor. She also has a very large goiter on the left side of her neck the size of a large grapefruit. CT scan says this mass displaces the trachea to the right significantly and causes "moderate" airway narrowing. It is thought to be a benign thyroid mass. Does this patient have pancreatitis?
Have a patient who had N/V, diarrhea and epigastric pain. CT scan with contrast showed diffuse mild thickening of colon and small bowel representing enterocolitis; pancreas had no abnormality. Lipase 7000s and patient did have epigastric tenderness on exam. Is this consistent with pancreatitis or does the ct scan rule it out? (Source: Student Doctor Network)
