Here’s some advice from a senior medical director from a leading prior auth company. You know, those guys who make you wait one week to do a P2P because you wanted to do SRS on a solitary brain met and the nurse reviewer said she can authorize it right away if you do it “by 3D.” Then the P2P guy says, sorry prostate cancer brain mets are excluded from our SRS policy. Early stage NSCLC SBRT/hypofrac
Wanted to review how you guys would approach this Got an elderly guy, GOLD 4 COPD, FVC 1.8L, 45% predicted, FEV1 860cc, 28% predicted, FEV1/FVC ratio 47%, with a peripheral early stage NSCLC, about 1.5cm, close to the chest wall, biopsy proven, PET negative, MRI brain negative. Hey guys, I would like some input about delirium management and organic mental disorders in general: after you order the inital labs and images, and there is for instance a brain lesion [tumor, stroke, etc], do you call Neurology and sign off the case, or do you work together with them? Fractionated vs. single fraction SRS for brain mets... What's your cut-off?
I am tending to fractionate tumors I can't get 20/1 into or at least 18/1. Under 2 cm/3cc or so I'll do 20/1 2-3cm / 3-14 cc I'll do 18/1 Towards the upper end of that range, I start thinking more about fractionated SRS, and > 14-15 cc I go 27/3. Not doing any single fraction < 18. Is anybody else? I don't like that grey area very much, and can't find a lot of good data on this. Curious how everyone else is approaching these 2.5 - 3.0 cm borderline tumors. And also for fractionated... Fractionated vs. single fraction SRS for brain mets... What's your cut-off? Anyone else enjoy undergrad coursework way more than med school coursework?
Hey everyone, Been studying all day, and came to a realization. I feel like medical school is 90% memorization. I literally spent all day learning about the minutiae that separates the different brain tumors. The day before, I spent all day memorizing and watching sketchy to learn the autonomic drugs. The day before that I was memorizing drugs to treat gout etc. I feel like theres very little understanding involved, just know certain things. Heck, I feel like my professors call things... SRS for BS lesion: what dose?
young female, known to have a metastatic breast cancer, since 5 years. In 2017, she developed secondary lesions in the brain with positive CSF. She receives 30Gy/10 fr whole brain radiation, with intra-thecal chemotherapy. A month ago, she developed gait disturbances with blurred speech an hemifacial paresia.
