Safe medication to give for someone who may be bipolar vs MDD?
Hi,
I am now starting outpatient in my third year and I have noticed that some of the patients I inherited are on SSRI's with a diagnosis of Bipolar Disorder. I was always under the impression that you avoid SSRI's without a mood stabilizer. My guess is these patients were probably Bipolar in the past but now they are being treated for depression as the Bipolar symptoms went into remission.
I was wondering though, for discussion sake, are atypicals and Lithium generally safe for someone...

How Would You Treat Bipolar Mania and Anxiety
Here's an interesting theoretical case running through my head:
Middle-aged man with diagnosis of bipolar with acute mania and anxiety. No other comorbidities. No allergies. Normal labs. This is your first time seeing the patient. How would you treat pharmacologically? If using benzodiazepines, what would you give for maintenance for anxiety?
Some possibilities from the top of my head:
- treat bipolar only and hope anxiety goes away once mania subsides
- give monotherapy that treats...

Unorthodox Use of Antipsychotics in Bipolar Disorder
Two questions:
1. I was reading UptoDate (Bipolar disorder in pregnant women: Treatment of mania and hypomania) and saw the following:
"The efficacy of haloperidol for treating mood elevated syndromes appears to be comparable to risperidone and olanzapine, and superior to quetiapine and lithium."
None of the attendings I work with prescribe typical antipsychotics for mania. Do you have experience doing so in non-pregnant patients? What was the result?
