At my job I've inherited some of the more complex cases after someone in my hospital system left. It is a population of veterans who get intensive case management and a number of them are diagnosed with treatment resistant schizophrenia/bipolar disorder. Classically, I was taught that if someone fails two antipsychotic trials, it is not unreasonable to consider clozapine and there is not much evidence for multiple antipsychotics. Had a patient come to me with a history of what sounded like a manic episode (with no prior history of mania), while taking double the maximum recommended dose of escitalopram. Safe medication to give for someone who may be bipolar vs MDD?
Hi, I am now starting outpatient in my third year and I have noticed that some of the patients I inherited are on SSRI's with a diagnosis of Bipolar Disorder. I was always under the impression that you avoid SSRI's without a mood stabilizer. My guess is these patients were probably Bipolar in the past but now they are being treated for depression as the Bipolar symptoms went into remission. A friend of mine just had his world flipped on him. His girlfriend of 4 years, with whom he has 2 children with, just left him for another man. Now, this is normal. Happens all the time. How Would You Treat Bipolar Mania and Anxiety
Here's an interesting theoretical case running through my head: Middle-aged man with diagnosis of bipolar with acute mania and anxiety. No other comorbidities. No allergies. Normal labs. This is your first time seeing the patient. How would you treat pharmacologically? If using benzodiazepines, what would you give for maintenance for anxiety?
