Fun Female Patient
I just picked up a 38-year-old female with H/O childhood emotional and physical abuse. Mood swings since teen years. She has been dx with Bipolar II disorder but I think she is Cyclothymic with seasonal pattern. She gets more depressed in fall and winter. She seems to only be slightly hypomanic with slightly less desire for sleep, no impulsive spending, hospitalizations, or legal problems. Just talks a little faster and with her hands, happier, gets more work done. When depressed just wants... Fun Female Patient (Source: Student Doctor Network)

Unreal.
Just letting you know it's a bit of a venting post. So, I had an intake yesterday, the guy had classic symptoms of borderline personality disorder, drinks a liter of liquor daily. He demanded to be diagnosed with bipolar disorder and be started on meds for that. I got him admitted for inpatient detox. History gathering showed no major mood symptoms in absence of drinking. I got inpatient doc to call me, he did not bother to read my notes. Didn't bother to even treat the etoh use disorder... Unreal. (Source: Student Doctor Network)

When mental illness gets personal - bridges burnt
Here is a posting some of you might find clinically interesting. Not the first either, as I can see. I suffer from bipolar disorder that I developed in graduate school when I was doing my MA in general psych. Motivated to overcome my challenges and succeed professionally, I decided at the time to continue pursuing my aspirations and finish my degree. I had goals of pursuing my PhD and making it in the field as a scientist-practitioner. I had a specific research interest. I graduated from... Letter of Recommendation advice please??
Okay so last semester I studied abroad and interned at mental health outpatient facility for individuals with schizophrenia, bipolar disorder, and unspecified personality disorders. It was a really great experience and my supervisor really really liked me. I asked him to write me a letter of recommendation for an REU this summer and he agreed. I emailed him about it on like December 25th, just sending him my resume, my research interests, transcript, and link to submit the letter; however,... Letter of Recommendation advice please?? 33, bipolar 1 with psychotic features, lawyer interested in going back to school to research mania
I’m wondering what the move is if I would like to be in a position to work directly on experiments to prevent my manic episodes without the aid of drugs like lithium. I wish there was just a facility I could make a living showing up to everyday and having tests done on myself, but seeing as that doesn’t exist, I’d like to figure out the best way to get in the game. Is it Med school? Doing a PhD and becoming some biomedical researcher? (Source: Student Doctor Network)

33 yr old, bipolar, lawyer, interested in PA and MD
Hey guys — I don’t have any prerequisites and my healthcare experience has just been from the jails/hospitals I’ve suffered during manic episodes. I guess that, combined with the fact my law career was sabotaged my health is why I’m interested in healthcare. I think it would be cool to be in a position to experiment on myself to end manic episodes with minimal side effects instead of just staying on lithium and hating my life. During my episodes I got both hands tattooed and the sides... 33 yr old, bipolar, lawyer, interested in PA and MD (Source: Student Doctor Network)

Grade/Curve?
Hi, I took an upper division psychology in the area of pathology (transnational relationship with family, relationships, and disorders i.e autism, depression, bipolar etc.) This class in the past 8 years thought by the same prof. the curve was 33-40% of students earn an A depending on the year. The syllabus did not contain what percentage was required to earn this grade, just a break down of the percentage of exams, assignments and participation. In my year, the curve was that 10%... Kratom and Opioids
We have a local naturopath in the area who is recommending a lot of Kratom to patients--advocating it for opioid withdrawal, depression, etc. How do you handle Kratom? Treat it like a herbal remedy or an ilicit botanical (cannabis, cocca teas, etc)... frustrated
Just need to vent. An NP in my practice left. Taking over her patients and feeling overwhelmed with the over diagnosis of bipolar disorder, ADHD and indiscriminant use of stimulants, benzos and antipsychotics. Seeing around 4 of her previous patients per day and totally exhausted dealing with this. There should be more regulation on the use of these medications. (Source: Student Doctor Network)

Complex Regional Pain Syndrome
I have an interesting case I am dealing with right now. 36 y/o male with history of bipolar disorder, depression, ADHD. This Gentleman has a 4 month history of bilateral heel pain (R > L). Before he presented to me he had been dealing with this for two months on his own with no home treatments. Started treating him in late August 2017. He works as a truck driver and stands for long periods of time in boots. He initially presented with symptoms consistent with bilateral plantar fasciitis and... Neuropsychiatry fellowship at Psych Dept
I matched into a neurology residency, but I also really like working with schizophrenics and bipolar I patients. If i completed a one or two year neuropsych fellowship at psychiatric hospital, would it be possible to learn how to manage patients with psychotic disorders and potentially run a unit as a physician with a residency in neurology? My interests may change in the future, but I was just curious if anyone thinks this is actually possible? (Source: Student Doctor Network)

Bipolar II vs. malignant narcissism
There's a lot in the literature about bipolar/borderline PD diagnostic issues (some arguing that they're on the same spectrum), but I find just as many patients who straddle the line between narcissistic PD and bipolar, type II. The typical case is the relatively successful executive who suffers from bouts of "depression," often with irritability and lability, some alcohol use thats minimized, that often coincides with problems in their business or relationships. During this period,... Bipolar II vs. malignant narcissism (Source: Student Doctor Network)

Depakote
Hi, I have a patient with Bipolar Dx who is doing beautifully on Depakote. However, she is concerned about her weight gain (she's been taking it for a few months). She is taking 750 mg of the Delayed Release. I would like to taper it off and start her on Lamictal or Topamax. I was wondering generally, how do you guys taper Depakote to discontinue? I hate taking the medication away as she is doing so well. Thanks (Source: Student Doctor Network)
