What is arterial switch operation for TGA? Cardiology Basics
Arterial switch operation is the ideal corrective surgery for transposition of great arteries (TGA). It was described by Jatene and colleagues in 1976. Normally aorta originates from the left ventricle and pulmonary artery originates from the right ventricle. This is reversed in TGA, so that pulmonary artery arises from the left ventricle and aorta originates from the right ventricle. Arterial switch operation normalizes this relationship, by moving the aorta and pulmonary artery back to their expected positions. When the blood vessels are switched, the coronary arteries are also repositioned to arise from the new aorta w...

Heart disease in pregnancy? Cardiology Basics
Changes in blood circulation during pregnancy and labour can adversely affect many of the significant heart diseases. Increase in blood volume and heart rate are the important factors during pregnancy. Increase in blood volume is needed to give enough nutrients and oxygen to the growing baby. Nutrients and oxygen are transferred to the baby through the placenta during pregnancy, though there is no actual mixing of the blood of the baby and mother. In general, obstructive lesions and complex cyanotic congenital heart diseases have high risk in pregnancy. Obstructive lesions like severe aortic stenosis and mitral stenosis a...

What is TAPVC? Cardiology Basics
TAPVC stands for total anomalous pulmonary venous connection. It is also known as TAPVD or total anomalous pulmonary venous drainage. Normally, pulmonary veins join the left atrium. When it drains to the right atrium through one of the blood vessels leading to the right atrium, it is known as TAPVC. If only some of the four pulmonary veins join the right side of the heart, then it is called PAPVC or partial anomalous pulmonary venous connection. When all the pulmonary veins join the right side of the heart instead of the left side, an atrial septal defect (ASD) is needed to maintain life. Otherwise body will not get any o...

Health Care Execs Behaving Badly
BY KIM BELLARD In the midst of a pandemic during which health care workers proved themselves to be very bit the heroes we like to think of them as being, it’s sobering to be reminded that the system they work in is filled with perverse incentives that work against patients’ best interests.  Four pieces of excellent journalism – two from The New York Times, and two from Kaiser Health News — this week brought that front and center.   If you haven’t read them yet, I urge you to do so, but, while you might enjoy the writing, don’t expect to enjoy their content. Let’s start with two arti...

Precautions for cardiac catheterization in a cyanotic child
Pediatric diagnostic cardiac catheterizations have come down with the availability of better imaging modalities which can give most of the details needed for management non-invasively. Still cardiac catheterization may still be needed in selected cases, leave alone the catheterization prior to pediatric cardiac interventions. Cyanotic child presents more challenges during cardiac catheterization, mostly due to the presence of hypoxia and polycythemia. Hospitalization on the previous day will be desirable along with 3-4 hour fasting prior to procedure. As there is a chance for hemoconcentration during fasting, adequate hyd...

Pneumatic Assistive Device for People with Disabilities
Researchers at Rice University have developed a pneumatic assistive device for people with disabilities. The technology includes an air pump that is mounted in the wearer’s shoe, providing pneumatic power with each step. This power is stored in a wearable belt that includes an “arm” that can reach out and grip items when activated. The device may be very practical for people with arm weakness who struggle to lift objects. The research team also developed a shirt with a bellows mechanism in the armpit that lets a wearer pick up an object that weighs 10 pounds. The researchers are exploring the possibility of u...
