Treating COVID pre and post intubation
Anecdotally it seems like we have two categories of treatments. 1. Hydroxychloroquine +/- azithromycin. 2. Xeljanz/Anakinra/Tocilizumab. The first category is treating the virus and superimposed bacteria. The second category is treating the cytokine storm and quite useful for when the patient is in ARDS. I think it's a reasonable school of thought that the first category should be used early (outpatients and early inpatient) and the second category should be the primary treatment modality... What do you do?
Patient presents to your office with a productive cough and occasional chest pain. Sats 90%, cxr shows LLL pneum. Confer with associate, admit to hosp. U note allergy to augmentin as it causes sob and facial swelling. Admitting orders keep sats >92%, Azithromycin 500 iv today and 250 dly, cbc cmp hscrp day, IV d51/2 ns @ 125/hr, rocephin 1 gram q 12h-aware pcn allergy, toradol iv 15mg q6h prn pain, ekg, trop. long term linezolid
Hi, I'm new to this forum. I don't have cf, I have bronchiectasis, but I have had mycobacterium abscessus for 2 years, and intermittent pseudomonas, so many of the threads here are very relevant and useful to me. I've had various iv regimes and am currently on imepenem and tigecycline with azithromycin. I specifically want to ask for experiences people have had using linezolid. I've had it for short periods ( 4 weeks at 600 mg a day) and sense that it helps ( sensitivity is intermediate) My physician is very cautious about side effects (which I like) and he's reluctant for me to h...

Bleeding - Rifampin???
I am currently on Rifampin, ethambutol and Azithromycin to treat MAC. Over the last two weeks I have had blood in my stool 3 times, bright red. Rifampin is known to cause bleeding as a side effect. Just found out daughter has Mycobacterium Abscessus
We just found out my daughter 11 has mycobacterium Abscesses (not MAC which she has also cultured in past but now apparently clear) and will start the treatment process soon. right now they r recommending IV Amikacin, IV imipenem, and azithromycin for at least 5 weeks (up to 3months) until cultures clear. Her pfts are still in the 80s/ low 90s, but it's still a drop from her baseline of over 100 last year. Do you have any advice as we start this journey? Drugs During Pregnancy
I'm sure this answer is here somewhere but what drugs are allowed and not allowed during pregnancy. I am currently on TOBI Podhaler and alternating that with cayston. Antibiotics and birth control
Hi All, I know antibiotics can interfere with birth control and make it less effective and even fail. But I am just wondering which antibiotics exactly do this and to what extent? Is it inhaled such as Tobi (taken every other month) or the pill antibiotics such as azithromycin ( I take 3 times a week)? I always want to be safe in preventing pregnancy but this worries me. Macrolides & Quinolones and cardiovascular risks
