Thoughts on ADHD stimulant Rx with UDS positive/Cannabis Use?
I've spoken to a lot of psychiatrists, and read a lot of papers online past few days. Seems to be a lot of back and forth, whether or not reasonable to prescribe medications like Adderall or Ritalin for patients that use cannabis, particularly recreationally. Some state no way, others state treatment of ADHD obviously helps with substance use. Thoughts, or any good links for further reading? (Source: Student Doctor Network)

Whats your approach to controlled substances?
When is comes to Testosterone, sleep (zolpidem/lunesta/etc), anxiety (BDZ), ADHD meds, opioids... Do you prescribe these? If no, why? Do you see these patients every 3 months/6 months/annually prior to continuing refilling these meds? Require specialist check in annually (ie. Psych with ADHD)? Do you have a contract with all or just specific drugs? And any other information you wanna share Just curious what other providers' routine is when it comes to these meds. Thanks. Husband is MD, scripts controlled meds to Wife
You have a stable inherited outpatient, being seen primarily for ADHD, on Adderall 10 mg BID. She mentions her husband is a physician. After leaving the appointment, you check the PDMP again and realize that she's gotten a few scripts of Adderall and short-acting benzos for insomnia from him several times (field unrelated to psychiatry). Nothing majorly concerning from the fills, appears like it was bridging refills and/or sporadic, nothing majorly concerning for abuse. Clown ****. What... Read more (Source: Student Doctor Network)

Need help, can I still get into med school with my situation?
So here's the deal: I want to become a psychiatrist. I completed community college for the first two years and then transferred to complete the last two (I took my sister's advice who attended a four-year college but regrets not completing her first two at a community college). I was a very ****ty high school student due to ADHD (for those of you who have it, you'll probably relate to not being able to read or do work without becoming easily distracted or wanting to do something else, for... Patient reviews should be banned in psychiatry.
Sorry im just ranting but lately we have implemented a system where patients leave ratings for each category based upon their experience. So of course the people who leave reviews are the drug seekers/severe personality disorder people who didnt get what they want. I feel like we have plunged into the era of "the patient is always right!", so its like Fk knowledge, actual facts, or data because 70 year old woman has new onset ADHD and needs her adderall. Last 5 intakes were PCP referring... Read more (Source: Student Doctor Network)

Any Students with Learning Disabilities?
Anyone here have a learning disability? How are you coping? I am a first year vet student and feel like I won't ever be able to keep up with the workload. I have autism, adhd, and dyslexia. Anatomy is killing me because I can't pronounce or spell any of the structures so its really hard to be able to study. In high school and undergrad I usually had to study at least double what the average person did, but with the intense workload there's not enough hours in the day to do that. I do have... Does anyone bill for psychological testing?
I use the CAT-A for all my ADHD intakes. I've just been eating the cost and considering it part of my overhead. However, I was wondering if I could bill one of the psychological testing codes for this? 96136? 96130? (Source: Student Doctor Network)

Earning Potential For Assessment Focused Practitioners?
Just curious about the earning potential for people focused on assessment. I talked to someone today who mentioned how the earning potential is high. I'm familiar with the Sweet survey and Neuropsych earning potentials, but I'm curious about personality, intellectual, ADHD, behavioral, etc assessments. There's probably lots of factors that go into it, but a general idea would be neat. But some example questions: What's the usual pay per assessment? (Or general range for all of them?) How... Using vyvanse plus adderall IR for ADHD?
I have an intake this week for a woman in her 30s, coming for ADHD management. Looks like shes been getting vyvanse 70mg qam and adderall IR 10mg on top of that. is there any evidence for using adderall IR in addition to vyvanse? Vyvanse tends to last a while, and if anything maybe mydayis would be a consideration if the issue was that it wore off but would you guys feel comfortable maxing out vyvanse and giving someone adderall IR on top of this? Most of my patients for ADHD tend to do just... Read more (Source: Student Doctor Network)
