Commentary: Squamous Cell Carcinoma of the Tongue in Young Patients: A Case Series and Literature Review

The recent article by Best et al provided a broad analysis of the complexities of treating squamous cell carcinoma and oral mucosal dysplasia in young patients.1 One of the major questions that exist in treating these patients is when to excise a dysplastic lesion. While the recommendations in treating moderate to severe dysplasia is excision, there is a gray area that exists when treating mild dysplasia: do we monitor the lesion closely for signs of progression, or go directly for excision? The transformation of mild dysplasia is around 5% from the reported literature, well below the extremes for moderate (up to 15%) and severe (up to 50%) dysplasia.
