Apixaban Level and Its Influence on Immunosuppression and Graft Outcome in Kidney Transplant Recipients With Atrial Fibrillation

Conclusions: Similar to nontransplant patients, KT patients administered the standard 5-mg bid dosage had apixaban levels that were well within the recommended manufacturers' expected ranges. In addition, this dosage had minimal influence on immunosuppression and no effect on graft function.

A 55-year-old lung transplant recipient with hypertension, atrial fibrillation, and prior steroid-induced hyperglycemia presented with choledocholithiasis and subsequently developed cholangitis with septic shock requiring intensive care unit admission after stone extraction. She developed progressive oliguric acute kidney injury (200 ml urine output in 24 hours), which necessitated the initiation of continuous venovenous hemodialysis because of her respiratory status (blood flow 300 ml/min; dialysate flow 28 ml/kg/h [2000 ml/h]; [bicarbonate] 32 mEq/l; [glucose]  = 100 mg/dl).
