Portal venous gas and hepatic cyst infection with Clostridium perfringens

An 80-year-old man presented to the emergency department with acute abdominal pain. He had a history of hepatic cysts incidentally discovered 4 years earlier during a workup for mild thrombocytopenia. He later developed mild alkaline phosphatase elevation attributed to cyst growth. Laparoscopic drainage of his dominant, 16 cm, right lobe cyst was considered but ultimately deferred given he remained asymptomatic. Conservative follow-up with serial laboratory and imaging studies was planned.
