Vitamin C for Sepsis and Acute Respiratory Failure

To the Editor The CITRIS-ALI trial revealed no difference in the primary outcomes of organ failure and biomarkers of inflammation and vascular injury in patients with sepsis and severe acute respiratory failure randomized to intravenous infusions of vitamin C or placebo. However, unadjusted analyses of predefined secondary outcomes suggested a possible benefit of high-dose intravenous vitamin C on mortality. Given that a before-and-after study suggested a mortality benefit among patients with sepsis receiving intravenous vitamin C, thiamine, and hydrocortisone, we believe the differences in mortality observed in the CITRIS-ALI trial may cause some practitioners to be more accepting of high-dose intravenous vitamin C.
