Rapid spontaneous resolution of lumbar ganglion cysts: A case report.

CONCLUSION: Spontaneous resolution of lumbar ganglion is very rare and only 26 cases, including ours, were reported in literature. Different degrees of biomechanical impairment seem to play a fundamental role in the pathogenesis. Related symptoms are essentially represented by low back and/or radicular pain, without significant neurological disorders. Anti-inflammatory drugs, light unloading exercises and brace could be recommended to administrated pain and decrease facet loads. Mean time for clinical improvement was 7 mo, while MRI disappearance occurred in an average time of 11 mo. Therefore, surgery should be applied when conservative treatment, prolonged at least 6 mo, fails. PMID: 31966971 [PubMed]

We present the case of a 48-year old male patient with chronic motor complete SCI, who benefited from a 5-day period of bilateral L2-level DRG-stimulation by experiencing suppression of transfer-evoked spasticity problems and of chronic lower back pain. To the best of our knowledge, this is the first case describing the successful application of DRG-stimulation for spasticity depression in patients with chronic SCI.

CONCLUSIONS: SRS did not alter sensory or motor function but reduced the activation of satellite glial cells, a pathway for DRG-mediated pain perpetuation. Radiosurgery provoked changes equivalent to the effects of focal radiation on the trigeminal ganglion after SRS for trigeminal neuralgia, suggesting that radiosurgery could be successful in relieving radiculopathic pain. Neuropathic pain is a very common clinical disorder, usually caused by nerve injury or accompanying some diseases (such as diabetes). It reduces the quality of life in patients due to a lack of effective analgesic strategies. The altered gene/protein expression in primary sensory neurons following injury has been demonstrated as the key component of the pathophysiology of neuropathic pain 16,17,29,40,41.
