We report a patient with relapsing-remitting multiple sclerosis, who developed rheumatoid arthritis after exposure to natalizumab. While some multiple sclerosis therapies are known to unmask autoimmune conditions, natalizumab is rarely implicated as a cause of alternative autoimmunity. This case illustrates an unusual clinical scenario which may support recent scientific work suggesting that when natalizumab blocks T helper 1 cells from entering the central nervous system, T helper 17 cells may continue to migrate into immune-privileged spaces and cause pathologic inflammation.Brief Background: Multiple sclerosis (MS) pati...

Robin shared her husband’s life-altering Wheat Belly experience: “I wanted to encourage those just starting out and getting through the difficult detox. If you are rethinking your decision, know that you are doing the best thing you could ever do for yourself and your family. “This is a picture of my husband, Kirk. He is 50 and had plans of having to go on disability this year due to the extreme pain in every joint in his body. Walking was extremely difficult. He works 10 hours a day in lawncare. AbstractPsoriatic arthritis (PsA) is an inflammatory arthritis with distinct phenotypic subtypes. Enthesitis is assigned as a hallmark of the disease, given its significant relations to disease activity and quality of life. Our objective is to evaluate the prevalence of enthesitis and its association with some clinical parameters, particularly quality of life, using data from a national registry. Patients with PsA meeting ClASsification criteria for Psoriatic Arthritis (CASPAR) were enrolled by means of a multi-centre Turkish League Against Rheumatism (TLAR) Network Project. The following information was recorded in web-ba...

ConclusionCitral is an effective analgesic in an arthritic pain model by increasing spinal 5-HT signaling and reducing spinal SAPK/JNK phosphorylation and systemic oxidative stress.Graphical abstractNeurochemical mechanisms involved in citral-induced analgesia in arthritis. (A) Complete Freund's adjuvant (CFA)-induced arthritis reduces spinal serotonin metabolism in the lumbar dorsal horn of the spinal cord ipsilateral to the injured joint, as indicated by the reduction in the spinal levels of the main metabolite of serotonin (5-HT), 5-hydroxyindole-3-acetic acid (5-HIAA). Moreover, there is a reduction in glycogen synthas...

ConclusionsCrowned dens syndrome should be considered, and craniocervical junction imaged in the context of acute cervical or occipital pain with stiffness and elevated inflammation markers not only in patients previously diagnosed with calcium pyrophosphate dehydrate arthropathy but also in diverse clinical settings.Key Points•This report highlights that crowned dens syndrome should be considered in various clinical setting besides calcium pyrophosphate dehydrate (CPPD) arthropathy.• Vigilance to this syndrome allows rapid treatment and may spare the patient unnecessary invasive procedures (i.e., temporal artery...

ConclusionThe definite diagnosis was not defined via vigorous myopathic and rheumatologic investigations. Detailed clinical examination and skeletal survey, followed by genotypic confirmation, were our fundamental pointers to rule out the false diagnosis of juvenile rheumatoid arthritis and rheumatoid polyarthritis in the adult group of patients. We wish to stress that the clinical/radiological phenotype is the baseline tool to establish a definite diagnosis and to guide the geneticist toward proper genotype.Key Points•Joint pain and difficulties in walking/climbing the stairs are characteristic features encountered i...

Conclusions: The use of KP to treat OVCF in RA group exhibited similar outcomes to non-RA group in terms of pain reduction, vertebral height restoration, and kyphosis correction. However, RA group had significantly higher rate of complications involving adjacent segment fracture and recollapse. Therefore, careful followup after KP in patients with RA is required to monitor for high complication rate.

If you’re like most people, you’ve seen a ton of direct-to-consumer (DTC) drug ads in recent years. They’re all over television, in magazines, online, on billboards, and slapped on the sides of buses, promoting treatments for arthritis, cancer, heartburn, psoriasis, flagging memory — and more. The deluge of drug ads can be overwhelming. Worse, the information is often incomplete, biased, or confusing. That’s why we’re launching the Harvard Health Ad Watch series to highlight some benefits and problems with health product advertisements. We’ll focus on the evidence behind the ads an...

ConclusionsMore than half of Medicaid beneficiaries receiving an opioid for pain management do so for orthopedic- and dental-related reasons, with emergency department providers more likely to prescribe opioids. Modifications to the guidelines addressing temporary acute pain management practices with opioids would be likely to benefit emergency department providers the most.
