Diabetic myonecrosis: a diagnosis to be considered

Diabetic myonecrosis is an under-recognized complication of long-standing poorly controlled diabetes mellitus usually involving the quadriceps muscle. The disease is self-limited with an excellent short-term prognosis but unfortunately a poor long-term prognosis because of macrovascular events. Herein, we present a case of diabetic myonecrosis and discuss the pathophysiology, presentation, diagnosis, and management of the disease. Physicians should maintain a high index of suspicion for this rare clinical entity in diabetic patients presenting with acute muscle pain.
