Interstitial nephritis in immune checkpoint inhibitor therapy

A 67-year-old woman with a history of metastatic melanoma on therapy presented to the oncology clinic with worsening fatigue for 2 weeks. She had subjective fevers, chills, and a pruritic rash. Her melanoma had progressed on pembrolizumab and so she had recently been started on combination therapy with ipilimumab and nivolumab about 3 months previously. Because of concerns for disease progression, she underwent a positron emission tomography –computed tomography (PET-CT) before being seen in the clinic.
