Abstract We investigated the effects of ibandronate, a bisphosphonate; eldecalcitol, an active vitamin D3 analogue; and combination treatment with both agents on secondary osteoporosis and arthritis using rats with adjuvant-induced arthritis. Arthritis was induced in 8-week-old male Lewis rats. Rats were randomized into four treatment groups and an untreated normal control group: ibandronate, eldecalcitol, ibandronate + eldecalcitol, vehicle, and control. Paw thickness was measured to evaluate arthritis. Joint destruction was evaluated histomorphometrically by the ankle joint stained with Fast Green and safranin O...

Proximal junctional fractures (PJFr) can be a catastrophic complication associated with adult spinal deformity surgery. Presence of osteoporosis has been shown to be one of the risk factors for PJFr. Dural-Energy X-Ray Absorptiometry (DEXA) scans are standard screening tools for diagnosis of osteoporosis. However, spinal DEXA scores have been reported to be flawed with regional sclerosis associated with spinal degeneration representing a possible confounder to the study. Recently, investigators have reported on the use of a computed tomography (CT) scan surrogate in place of spinal DEXA scores.

Teriparatide has been used to improve bone quality when considering spinal fusion surgery in patients with osteoporosis. Hounsfield units (HU), a standardized measurement for radiodensity, can be obtained on routine computed tomography (CT) scans, while dual energy X-ray absorptiometry (DEXA) is the common test for bone density.

AbstractObjectivesTo develop and evaluate a method for areal bone mineral density (aBMD) measurement based on dual-layer spectral CT scout scans.MethodsA post-processing algorithm using a pair of 2D virtual mono-energetic scout images (VMSIs) was established in order to semi-automatically compute the aBMD at the spine similarly to DXA, using manual soft tissue segmentation, semi-automatic segmentation for the vertebrae, and automatic segmentation for the background. The method was assessed based on repetitive measurements of the standardized European Spine Phantom (ESP) using the standard scout scan tube current (30  ...

[Hounsfield units as a measure of bone density-applications in spine surgery]. Unfallchirurg. 2019 May 03;: Authors: Scheyerer MJ, Ullrich B, Osterhoff G, Spiegl UA, Schnake KJ, Arbeitsgruppe Osteoporotische Frakturen der Sektion Wirbelsäule der Deutschen Gesellschaft für Orthopädie und Unfallchirurgie Abstract Despite today's good diagnostic and therapeutic options for osteoporosis, the number of unidentified cases is very high and therapy is therefore usually inadequate. Frequently, the diagnosis of osteoporosis is made only after the occurrence of a fracture. The reason for...

AbstractPurposeTo evaluate the diagnostic performance of Hounsfield unit histogram analysis (HUHA) of precontrast abdominal-pelvic CT scans for predicting osteoporosis.Materials and methodsThe study included 271 patients who had undergone dual X-ray absorptiometry (DXA) and abdominal-pelvic CT within 1 month. HUHA was measured using commercial 3D analysis software (Aquarius iNtuition v4.4.12Ⓡ, TeraRecon) and expressed as a percentage of seven HU range categories related to the ROI: A

ConclusionCT offers a combined opportunistic screening for osteoporosis by assessing both vertebral fractures and bone density on routine CT-scans. This approach may be particularly interesting for RA patients with a high osteoporosis risk.
