Perils of bridge to transplantation in patients on continuous-flow left ventricular assist devices

Although overall heart transplantation outcomes and survival have clearly improved over the last several decades, the concomitant outcomes with primary graft dysfunction (PGD) have been less defined. The ISHLT Consensus Conference laid down the framework toward a common definition of PGD and highlighted the historical and contemporary variability in reported incidence across institutions.1 In the current era of increasing numbers of patients on left ventricular assist device (LVAD) support, it seems that some centers have experienced an increase in the incidence and severity of PGD.
