Under which cardiac disease spectrum should takotsubo syndrome be classified?

I read with great interest the proposal raised by Pelliccia et al. [1], in the recent issue of the International Journal of Cardiology. The authors have provided evidences that takotsubo syndrome (TS) is not a cardiomyopathy and proposed the inclusion of TS within the spectrum of ischemic heart disease. This author agrees utterly with first suggestion but disagree with the second proposal. Long-lasting ST elevation myocardial infarction-like ECG-changes with extensive left ventricular wall motion abnormality and mild elevation in infarction biomarkers challenge any kind of coronary ischemia including micro-vascular spasm or dysfunction causing TS.
