Partnering4Health Report

Partnering4Health is a project funded by the Centers for Disease Control and between 2014 and 2017. Five national organizations were provided with CDC funding to support communities in their efforts to promote healthy lifestyle. The final aim of this project was to reduce death and disability due to tobacco use, decrease obesity and reduce the rate of death and disability related to diabetes, heart disease, and stroke. Overall, 94 communities were provided with funding to make healthy food more accessible, create opportunities for physical activity, promote smoke free spaces and create breastfeeding friendly environments. The report National Organizations Empowering Communities to Improve Population Health outlines the strategies and processes used during this project as well as results and recommendations.
