Conclusions—BOLD delay is related to macrovascular delay and microvascular hypoperfusion, can identify severely hypoperfused tissue in acute stroke, and is a promising alternative to gadolinium contrast agent–based perfusion assessment in acute stroke.Clinical Trial Registration—URL: http://www.clinicaltrials.gov.
