Letting Radiation Therapy “Make Its Bones”

For nonmetastatic unresectable pelvic osteosarcoma, we approach local therapy with curative intent to prevent the devastating consequences of local tumor progression, accepting that less than 20% of patients with unresectable disease are long-term survivors.1 Reports of success with definitive radiation therapy (RT) vary widely, but modern series have achieved 5-year local control exceeding 60% to 70% with definitive doses of proton RT (>65-70 Gy).2,3 Indeed, RT is the only local control modality mentioned by the National Comprehensive Cancer Network guidelines for unresectable osteosarcoma.
