HCC: We Prefer Fractionation Versus Dose Reduction
We recommend ablative doses with proton therapy for most patients with cirrhosis.1 The advantage of improved parenchymal sparing may lower the risk of radiation-induced liver disease and treatment-related liver failure.2 Instead of always giving 5 fractions and reducing the dose to a palliative level for larger volumes, we prefer to combine the technological innovations with the foundational principle of fractionation to deliver ablative doses of radiation therapy (Biologic Equivalent Dose>100 Gy).
