Wretches and Jabberers
“Dispel the darkness around us wretches. Take us for real people. Don’t sideline us.” –Tracy Thresher, “Wretches and Jabberers” The other night I had the opportunity to bring my 24 year old autistic son Nat to a dinner with Tracy Thresher and Larry Bissonnette, the stars of the documentary “Wretches and Jabberers,” directed by Gerardine Wurzburg. The film was the kickoff movie for an event called “ReelAbilities,” a mini film festival running January 31 – February 6, under the aegis of the Boston Jewish Film Festival. ReelAbilities is dedicated to showing movies that hav...

Overcoming EMR Implementation Barriers in Medically Underserved Areas
This post has been contributed by Alan Edwards who holds an MBA in Health Care and is a former hospital CEO and group practice administrator. He continues to be active in the field and often writes on health insurance and health policy. The world of medical care providers turned itself upside-down and inside out over implementation and meaningful use of electronic medical records also known as electronic health records. The industry uses the acronyms EHR or EMR to describe electronic medical records. Implementation of EMR is not part of the Affordable Care Act. Though President Obama is a major supporter of the EMR, implem...

Is the death of the solo practitioner a bad thing?
In recent years, there has been a shift away from solo practice to group practice among physicians. The trend, which some say began in the early 2000s, appears to have accelerated with new incentives proscribed in legislation such as the Affordable Care Act. The push for accountable care organizations seems to be fostering the rapid reorganization of medicine from one based on solo practitioners and small groups to one based on large, multi-specialty groups. Your mother was a 'amster
In DC with Patrick Bruce Jordan (third from left) and Tom Wahl (far right)I'm engaged to King Arthur. That's right...some day, some day I shall be Queen! No, that's not quite right. But Patrick flew down to Sarasota this week to start rehearsals for Spamalot. It's directed by the incomparable Bruce Jordan who does comedy, farce and hilarity with crowd-pleasing precision.I am so incredibly proud of Patrick and can't wait to see him in it when I go down there at Thanksgiving time.Since Patrick and I have been together I've discovered all the heart and soul that goes into in his craft and it's made me appreciat...

Nurses and Collective Success
Collectivism and collaboration are hot topics in the entrepreneurial spaces where I currently hang out. Just last weekend, my business partner and RN.FM Radio co-host Kevin Ross and I were at the National Nurses in Business Association annual conference in Orlando, and our presentation was entitled "Cross-Pollination: Collaboration on Steroids". This notion was well-received by the audience, and our assertion that competition belongs in the dustbin of history seemed to hit home with most everyone who was present. In high-quality healthcare environments, collaboration and cooperation are essential. Imagine an operating room ...

Doctor Explains Why He’s Avoiding EMR
Dr. Peter Kambelos likes being in solo practice because he can make all the decisions. And for now, the internist has decided to keep using paper records. It’s partially an economic decision. He doesn’t feel he can afford the switch to an EMR. Politics might play a bit of a role, too. Kambelos, who is president-elect of the Academy of Medicine of Cincinnati, likes to keep the government out of his exam room. Still, he doesn’t rule out going electronic in the not-too-distant future. After all, he’s a mid-career physician, and it would be absurd to think of sticking with analog charts forever. Late adopters like Kam...

FDASIA Report on Regulating HIT
On September 4, 2013, the FDASIA mandated workgroup presented their recommendations to the Health IT Policy Committee in Washington, D.C. Some reporting on the meeting cast the draft report as downplaying potential FDA regulation of healthcare IT applications (HIT), while others emphasized the uncertainty (subscription required) of the process and ultimate outcome. Such news stories are, by necessity, short and can’t cover all the issues but this one from iHealthBeat provides a good summary. The final report (links to all draft documents) was submitted September 4, 2013 and was basically unchanged from the preliminar...

Privacy and Security: How to Make Sense of all the Regulations
Wed, 8/21/2013 - 9:01amRob Brull0 Comments var addthis_config = {ui_click: true,data_track_clickback: true,data_ga_social: true,data_ga_property: UA-317164-6}; It seems like not a week goes by without some organization being fined in excess of a million dollars for some type of PHI (Protected Health Information) security breach. The latest breach this week has to do with a leased photocopier. Apparently Affinity Health Plan, a New York-based managed care plan, returned a leased photocopier that contained PHI on the hard drive for up to 344,579 individuals.   According to an article by Healthcare IT Ne...

A warm farewell and welcome to full time in education
Check out this lovely column about Robin Dibner, who is leaving medical practice to devote full-time to residency education.  One quote:When you are in an ambulatory practice as an attending, whether solo, group, specialty, or faculty practice – you and your patients choose each other.  Over time the ones who are not a good fit drift away.  The ones who remain loyal are wonderful!  And you learn to work with each one in a collaborative relationship to promote their health, adherence to treatments, and prevention.  They learn to trust you, and you them. Meaningful Use Will Force Doctors into ACOs
They have made this mu reporting so bad….as our doctor says…they do not want doctor’s to practice by themselves…They are doing all they can to get them into ACOS… The above is a quote from an EMR and HIPAA reader who emailed me about the meaningful use requirements. The conclusion was what really caught my attention. There are a number of questions that should be asked based on the statement above. Does the government want doctors to not practice by themselves? No one in government would say this and this isn’t their thought process at all. What they do want to see is a reform to how ...
