In an emergency, what can we do if “firemen” (trialists) come late to combat the covid-19 pandemic? What stops hypertonic saline for nasal rinsing and gargling from being an early, universal adjunctive weapon to combat SARS-CoV-2?

live and let die?
Is there any logic to simply allowing SARS-CoV2 to run its course? As pathologists, we are "the doctors doctor", and many of us are involved with COVID testing in our laboratories. However, we are not social engineers. And as physicians, we have to respect patient autonomy.
