Preoperative higher right ventricular stroke work index increases the risk of de novo aortic insufficiency after continuous-flow left ventricular assist device implantation
This study investigated the influence of preoperative hemodynamic parameters on de novo AI in CF-LVAD recipients. We reviewed 125 patients who underwent CF-LVAD implantation without concomitant AV surgery between 2005 and 2018. De novo AI was defined as moderate or severe AI in those with none or trivial preoperative AI. During mean 30  ± 16 months of CF-LVAD support, de novo AI-free rate was 86% and 67% at 1 and 2 years, respectively. In-hospital stroke and mortality trends after left ventricular assist device implantation in the United States from 2017 to 2019
CONCLUSIONS: Our study found a significant downtrend in the in-hospital mortality rates among patients with LVAD without substantial changes in stroke rate trends over the study timeframe. As stroke rates remained steady, we hypothesize that improved management along with better control of blood pressure, could have played an important role in survival benefit over the study time frame.PMID:37387231 | DOI:10.1177/03913988231183723

Detection of inflow obstruction in left ventricular assist devices by accelerometer: A porcine model study
Left ventricular assist devices (LVAD) provide circulatory blood pump support for severe heart failure patients. Pump inflow obstructions may lead to stroke and pump malfunction. We aimed to verify in vivo that gradual inflow obstructions, representing prepump thrombosis, are detectable by a pump-attached accelerometer, where the routine use of pump power (PLVAD) is deficient.

Elective Cardiopulmonary Bypass for Lung Transplantation: Is It Safer Than We Think
The use of cardiopulmonary bypass (CPB) during lung transplantation is controversial. CPB can offer improved visualization, better control of central vasculature, and possibly faster operations. Opponents argue the use of CPB results in higher transfusion requirements, increased rates of primary graft dysfunction (PGD), and more complications such as stroke, acute kidney injury (AKI) and renal failure requiring hemodialysis. In the fall of 2020 our program transitioned to performing all lung transplants on CPB.

Impact of Perioperative Sarcopenia and Dysphagia on LVAD Related Complications
Sarcopenia, as defined by decline in muscle quantity/quality, is prevalent in heart failure and associated with high morbidity and mortality. Sarcopenic dysphagia is caused by sarcopenia in the whole body and swallowing-related muscles. We investigated the impact of perioperative sarcopenia and dysphagia on postoperative pneumonia (PNA), stroke, gastrointestinal bleeding (GIB) and 30-d readmission after LVAD.
