Mediterranean-style diet and intracranial large artery stenosis in the Northern Manhattan Study
CONCLUSION: In this stroke-free subsample, we did not find a significant association between MeDi and ICAS. We may have been limited by statistical power.PMID:37634256 | DOI:10.1016/j.jstrokecerebrovasdis.2023.107252

Screening and Management of Coronary Artery Disease in Kidney Transplant Candidates
Diagnostics (Basel). 2023 Aug 20;13(16):2709. doi: 10.3390/diagnostics13162709.ABSTRACTCardiovascular disease (CVD) is a major cause of morbidity and mortality in patients with chronic kidney disease (CKD), especially in end-stage renal disease (ESRD) patients and during the first year after transplantation. For these reasons, and due to the shortage of organs available for transplant, it is of utmost importance to identify patients with a good life expectancy after transplant and minimize the transplant peri-operative risk. Various conditions, such as severe pulmonary diseases, recent myocardial infarction or stroke, and ...

Peripheral Vascular Disease and Carotid Artery Disease Are Associated with Decreased Bile Acid Excretion
This study investigated BAE in patients with peripheral vascular disease (PVD) and carotid artery disease (CA) and those without these diseases, compared to patients with CAD, stroke, or no evidence of atherosclerosis. Patients with complaints of chest pain-suspected CAD, syncope, stroke/TIA, severe headache, intermittent claudication, or falls were enrolled. All received a 4-day standard diet with 490 mg of cholesterol and internal standard copper thiocyanate. Fecal BAE was measured using gas-liquid chromatography. One hundred and three patients, sixty-eight (66%) men and thirty-five women (34%), mean age range 60.9 ± 8....

Update on Biomarkers Associated with Large-Artery Atherosclerosis Stroke
Biomolecules. 2023 Aug 16;13(8):1251. doi: 10.3390/biom13081251.ABSTRACTIntracranial and extracranial large-artery atherosclerosis (LAA) are a main cause of ischemic stroke. Biomarkers may aid in the diagnosis of LAA and help to stratify patients' risk of stroke. We performed a narrative review of the literature, mainly published in the last five years, with the aim of identifying biomarkers associated either with intracranial or extracranial LAA in humans. Several potential biomarkers of LAA, mainly related to lipidic pathways and inflammation, have been studied. Diagnostic biomarkers of LAA were evaluated by measuring bi...

Imaging Carotid Plaque Inflammation Using Positron Emission Tomography: Emerging Role in Clinical Stroke Care, Research Applications, and Future Directions
Cells. 2023 Aug 15;12(16):2073. doi: 10.3390/cells12162073.ABSTRACTAtherosclerosis is a chronic systemic inflammatory condition of the vasculature and a leading cause of stroke. Luminal stenosis severity is an important factor in determining vascular risk. Conventional imaging modalities, such as angiography or duplex ultrasonography, are used to quantify stenosis severity and inform clinical care but provide limited information on plaque biology. Inflammatory processes are central to atherosclerotic plaque progression and destabilization. 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) is a validated techn...

The Role of the Low-Density Lipoprotein/High-Density Lipoprotein Cholesterol Ratio as an Atherogenic Risk Factor in Young Adults with Ischemic Stroke: A Case-Control Study
This study investigated the TC/HDL-C ratio and the LDL-C/HDL-C ratio, which may be more specific and common lipid parameters in young patients with ischemic stroke. This study aimed to demonstrate the sensitivity and specificity of TC/HDL-C and LDL-C/HDL-C ratios as atherogenic markers for young adult ischemic strokes. This trial was conducted as a retrospective case-control study. A total of 123 patients (patient group) and 86 healthy individuals (control group) aged 18-50 years were randomly selected from four different hospitals. Lipid parameters and TC/HDL-C and LDL-C/HDL-C ratios were compared between these two groups...

Prevalence and Outcomes of Patients With Acute Ischemic Stroke and Concomitant Non –ST-Elevation Myocardial Infarction (Results from the National Inpatient Sample 2016 to 2019)
We examined hospitalized patients with acute ischemic stroke and a concomitant NSTEMI diagnosis who were included in the National Inpatient Sample 2016 to 2019. Acute ischemic stroke and NSTEMI were defined by using the Inter national Classification of Diseases, Tenth Revision codes. Patients with ST-elevation MI were excluded.

Outcomes of Concomitant Coronary Artery Bypass Grafting in Patients With Infective Endocarditis: A Systematic Review and Meta-Analysis
It is current practice to perform concomitant coronary artery bypass grafting (CABG) in patients with infective endocarditis who have relevant coronary artery disease. However, CABG may add complexity to the operation. We performed a systematic review and a meta-analysis of studies that presented outcomes from patients who underwent valve surgery because of infective endocarditis with or without concomitant CABG. Three databases were assessed. Perioperative mortality was the primary outcome. Long-term mortality and postoperative stroke were the secondary outcomes.

Long-term Incidence of Ischemic Stroke Following Transient Ischemic Attack: A Nationwide Study During 2014-2020
Conclusions: Patients with first-time TIA had an ischemic stroke incidence of 6.1% during the 5-year follow-up period. Following adjustment for relevant comorbidities, this incidence was approximately five-fold higher than what was found for controls in the background population, and 40% lower than for patients with recurrent ischemic stroke.PMID:37622531 | DOI:10.1161/CIRCULATIONAHA.123.065446

Ischaemic stroke despite antiplatelet therapy: Causes and outcomes
CONCLUSIONS: One-third of ischaemic strokes occurred despite APT and 20% of patients with a history of ischaemic stroke had no antithrombotic therapy when having stroke recurrence. Aetiology of breakthrough strokes despite APT is heterogeneous and these patients are at increased risk of recurrent stroke.PMID:37622482 | DOI:10.1177/23969873231174942
