Effect of the Pre-Operative Systemic Inflammatory Response on Outcomes Following Carotid Endarterectomy for Symptomatic Carotid Stenosis
Objective: Stroke accounts for over 100 000 acute hospital presentations per year in the UK, and approximately 4 500 carotid endarterectomies are performed annually in the UK.1 This is despite a relative paucity of cases performed for asymptomatic carotid stenosis, in comparison with other countries.1 Periprocedural stroke and death rates have fallen in recent years,2 although these remain notable risks necessitating careful patient selection and counselling. Chronic activation of the systemic inflammatory response (SIR) is prevalent in patients with atherosclerotic disease and influences both the pathogenesis and prognosis.

The Relationship Between Physician and Center Case Volume on Outcomes of TransCarotid Artery Revascularization
It has been suggested that the annual hospital volume of cases may affect the number of adverse events following carotid endarterectomy (CEA). Transfemoral carotid stenting (TFCAS) was found to have an extended learning curve with an estimated minimum of 25 cases for the success of the stenting procedure. One prior study indicated that transcarotid artery revascularization (TCAR) is being performed with excellent stroke and mortality rates even in the early stages of the surgeons ’ learning curve.

A Stepwise Protocol to Reduce Haematoma Rates in Carotid Endarterectomy
Introduction: Stroke is the leading cause of long term disability in developed countries, with symptomatic carotid artery stenosis causing 25% of ischaemic strokes. Timely carotid endarterectomy (CEA) for significant stenosis reduces the risk of further strokes. Haematoma is a complication of CEA, especially in patients on dual antiplatelet therapy. Haematoma can result in stroke, death, and prolonged hospital stay. To date there has been limited research on protocols that can prevent this complication.

Transcarotid Artery Revascularization Is Associated With Favorable Outcomes in the Treatment of Restenosis After Prior Carotid Endarterectomy
Restenosis after carotid endarterectomy (CEA) has been associated with an increased risk of ipsilateral stroke. Redo-CEA has a significantly higher rate of stroke and cranial nerve injury compared with primary CEA. Carotid artery stenting might provide a safer treatment option for restenosis. However, the optimal treatment modality for this pathology has yet to be determined. Here, we evaluate the in-hospital outcomes of transcarotid artery revascularization (TCAR), redo-carotid CEA, and transfemoral carotid artery stenting (TFCAS) in a large contemporary cohort of patients who underwent treatment for restenosis after CEA.
