Bypass and other modified reconstruction techniques for 'challenging' carotid cases: A comparison with conventional endarterectomy
CONCLUSIONS: Patients who underwent ICA revascularization with MCAR showed risks of perioperative death, major or minor stroke (<2%), reintervention rates and carotid restenosis rates that are comparable with PCEA or ECEA groups. Nevertheless, the MCAR group showed a significantly higher rate of global central neurological complications (considering together TIA, minor stroke and major stroke) than patients treated with standard CEA. Bleeding Issues During Thrombolysis for Acute Leg Ischaemia
When we started testing catheter directed low dose thrombolysis (CDT) for acute leg ischaemia (ALI) in the 1980s, it was the first time endovascular intervention advanced beyond simple balloon angioplasty. In many respects, CDT kick started the endovascular revolution. Clots could be removed without surgery, and from arteries too small for embolectomy catheters to reach. But there was a price to pay: thrombolytic drugs caused bleeding, mainly from catheter puncture sites, but also from distant areas, such as haemorrhagic stroke.

Nationwide outcome analysis of primary carotid endarterectomy in symptomatic patients depending on closure technique and patch type
CONCLUSIONS: There was an increased risk of ipsilateral stroke <30days in patients operated with primary closure compared to eversion-CEA and patch angioplasty. Outcomes and role of shunting during carotid endarterectomy for symptomatic patients
In this retrospective study of 4,652 patients with symptomatic carotid stenosis undergoing CEA and patch angioplasty, shunt placement was associated with two-fold increased risk of cranial nerve injury without reduction in postoperative stroke risk, irrespective of symptom severity.

Peri-procedural variables and outcomes of long-period hemodynamic instability after carotid artery angioplasty and stenting
CONCLUSIONS: CAS-induced HI occurs in a considerable percentage while several peri-procedural variables are determined as independent predictors to develop long-period HI. An alternative reduced dose regimen of ticagrelor for neuroendovascular patients
CONCLUSIONS: A lower dose of ticagrelor (45 mg twice daily) appears to be safe and effective in this small cohort of patients who are resistant to clopidogrel per P2Y12 testing and who have increased risk of ischemic or hemorrhagic strokes due to neurovascular pathologies and implants. Carotid Endarterectomy with Patch Angioplasty versus Primary Closure in Patients with Symptomatic and Significant Stenosis: A Systematic Review with Meta-Analyses and Trial Sequential Analysis of Randomised Clinical Trials
Introduction: Patch angioplasty in conventional carotid endarterectomy (CEA) is suggested to reduce the risk of restenosis and recurrent ipsilateral stroke, compared to primary closure. A systematic review of randomised clinical trials is needed to compare the outcomes (benefits and harms) of both techniques.
