Relation of Race, Apparent Disability, and Stroke Risk With Warfarin Prescribing for Atrial Fibrillation in Patients Receiving Maintenance Hemodialysis
We examined patterns of warfarin use, and associated factors, after AF diagnosis. This retrospective cohort analysis studied US Medicare patients receiving maintenance dialysis January 1, 2008-June 30, 2010. Demographics, comorbidity, and a durable medical equipment claims-based disability proxy score predicted warfarin prescription after AF diagnosis. The analysis included 8964 patients with non-valvular AF.

Racial/Ethnic Disparity Seen for Stroke in Dialysis Patients With A-Fib
MONDAY, Feb. 24, 2020 -- Among hemodialysis patients with atrial fibrillation, racial/ethnic disparities in all-cause stroke are partially mediated by lower anticoagulant use, according to a study published online Feb. 20 in the Journal of the...

Association between geriatric nutritional risk index and stroke risk in hemodialysis patients: 10-years outcome of the Q-Cohort study
The geriatric nutritional risk index (GNRI), which is calculated using the serum albumin level and body mass index, is a nutritional marker associated with an increased risk of cardiovascular events in patients who are receiving hemodialysis. However, no studies have examined the association between the GNRI level and the incidence of stroke in this population.

Impact of stroke history on the presence of cerebral microbleeds in hemodialysis patients
Cerebral microbleeds (CMBs) are detected on gradient-echo T2*-weighted magnetic resonance imaging (MRI). Clinically, CMBs are often detected after stroke, including in cases of intracerebral hemorrhage and isc...

Atypical presentation of fistula dysfunction due to brachial arterial embolization mimicking stroke
In this report, failure to recognize the upper limb ischemia would have led to delayed treatment, potentially resulting in the fistula's complete thrombosis and further limb ischemia. We highlight the importance of a diagnosis method like Doppler ultrasound, which allows for rapid evaluation at the patient's bedside.PMID:35000475 | DOI:10.1177/11297298211067686

Apixaban for stroke prevention in hemodialysis patients with non-valvular atrial fibrillation
Atrial fibrillation (AF) is a common arrhythmia among patients with kidney failure on maintenance hemodialysis (HD) and is associated with a high incidence of all-cause mortality and stroke in this population.1

Hypertension - human studies
Conclusions:Higher predialysis SBP variability is associated with stroke on HD populations suggesting a potential role for cerebral perfusion instability. We confirm known associations between age, diabetes, pre-existent cerebrovascular disease and hypertension. In contrast to studies in non-dialysis patients we do not find an association between EPO dose and stroke. Interventional trials of blood pressure management on stroke are recommended.

Prevalence and Etiology of Acute Ischemic Events During Hemodialysis (P1.076)
CONCLUSIONS: Over a third of stroke patients with ESRD/HD admitted over the study period had onset of ischemia during or shortly after HD. While clinicians may be tempted to dismiss neurologic changes after HD to metabolic etiologies, they should also be aware the HD represents a period of elevated risk for acute ischemia.Disclosure: Dr. Cherian has nothing to disclose. Strategy to a 5 Minute Door to Needle Time (S5.005)
CONCLUSIONS:Our experience suggests that the "Target: Stroke" strategies (EMS initiation of stroke codes, rapid triage, rt-PA before labs), SMART criteria, and three question tool to predict abnormal coagulation can significantly reduce the time to thrombolysis. However, a multi-disciplinary team, educated and dedicated to providing rapid ischemic stroke treatment, and the development of a culture in which faster treatment is a goal, is a factor that’s impact cannot be measured.Study Supported by:Disclosure: Dr. Parker has nothing to disclose. Operative strokes after repair of acute type a dissections: predisposing factors and implications.
CONCLUSIONS: Patients who developed stroke after type A dissection repair had higher complication rates and a higher mortality rate at 30 days, 1 year, and 5 years. Femoral cannulation was the only technical factor associated with a higher rate of strokes. PMID: 30747502 [PubMed - as supplied by publisher]

Can I use DOAC in a patient with renal disease?
Case A 76-year-old man is diagnosed with non-valvular atrial fibrillation. His comorbid conditions are hypertension, diabetes complicated by neuropathy, and chronic kidney disease stage 3. His current medications include metformin, lisinopril, gabapentin, and aspirin. His most recent laboratories showed a creatinine 1.8, creatinine clearance (CrCl) 35 mL/min, hemoglobin 11g/dL, and international normalized ratio 1.0. His congestive heart failure, hypertension, age, diabetes, stroke, vascular disease, and sex (CHADSVASc) score is 4. Which medication should we use to prevent stroke in this patient?  Brief overview of the is...

Racial Differences and Outcomes of Strokes in Patients on Hemodialysis: A 10-Year Retrospective Study (P1.081)
CONCLUSIONS: AIS was a common complication in HD patients affecting LOS as well as mortality. African Americans patients were younger compared to Caucasians, who had higher incidence of atrial fibrillation. Overall mortality was high and infections were an independent predictor of mortality in HD patients with stroke. Study Supported by: NoneDisclosure: Dr. Sunbulli has nothing to disclose.
